血清肿瘤标志物联合检测用于诊断非小细胞肺癌患者发生脑转移的效果分析

Analysis of the effect of combined detection of serum CEA,CA199,CA125 and CY21-1 on diagnosis of brain metastasis in patients with NSCLC

目的观察评估血清CEA、CA199、CA125、CY21-1水平的检测应用于非小细胞肺癌脑转移患者的临床诊断价值。方法选取2016年4月~2018年9月在首都医科大学附属北京潞河医院接受治疗的非小细胞肺癌(NSCLC)患者120例,根据其是否发生脑转移分为脑转移组(45例)和未发生脑转移组(75例)。检测并比较不同分期患者血清CEA、CA199、CA125、CY21-1水平的差异,分析上述指标联合检测对NSCLC患者的诊断价值。结果转移组患者的血清CEA、CA199、CA125、CY21-1水平分别为(25.78±5.36)ng/ml、(36.49±8.17)μg/ml、(58.86±10.36)μg/ml、(8.18±1.13)ng/ml,明显高于未转移组,差异有统计学意义(P <0.001);Ⅲ+Ⅳ期患者的CEA、CA199、CA125、CY21-1水平明显高于Ⅱ期患者(P <0.05);转移组患者上述指标的阳性率明显高于未转移组患者,差异有统计学意义(P <0.05);CEA、CA199、CA125、CY21-1联合检测对NSCLC脑转移患者的诊断灵敏度和特异度均高于单项指标检查。结论非小细胞肺癌脑转移患者的血清CEA、CA199、CA125、CY21-1水平较高,且联合检测对非小细胞肺癌脑转移患者的诊断价值更高。

Objective To observe the serum levels of CEA,CA199,CA125 and CY21-1 in patients with brain metastases from non-small cell lung cancer,and to analyze the clinical significance of the detection.Methods Patients with NSCLC who were treated in our hospital from April to May 20,2016 were divided into brain metastasis group and brain metastasis group according to whether they had brain metastasis.The serum levels of CEA,CA199,CA125 and CY21-1 were observed in the two groups.The differences of serum CEA,CA199,CA125 and CY21-1 levels in different stages were compared.The combined detection of serum CEA,CA199,CA125 and CY21-1 was performed on NSCLC.The diagnostic value of the patient.Results The levels of serum CEA,CA199,CA125 and CY21-1 in the metastatic group were 25.78±5.36 ng/ml,36.49±8.17μg/ml,58.86±10.36μg/ml and 8.18±1.13 ng/ml,respectively.In the metastatic group,the difference was statistically significant(P<0.001);the levels of CEA,CA199,CA125,and CY21-1 in patients with stage III+IV were significantly higher than those in stage II(P<0.05);CEA and CA199 in patients in the metastatic group.The positive rates of CA125 and CY21-1 were significantly higher than those of the non-metastatic group(P<0.05).The combined sensitivity of CEA,CA199,CA125 and CY21-1 was high in sensitivity and specificity for patients with NSCLC brain metastases.Check for individual indicators.Conclusion The levels of serum CEA,CA199,CA125 and CY21-1 in patients with non-small cell lung cancer brain metastases are higher,and the combined detection has higher diagnostic value for patients with non-small cell lung cancer.
