Lewisy抗原的表达与晚期卵巢癌患者化疗耐药的相关性研究

Study on the correlation between the expression of Lewis antigen and chemoresistance in patients with advanced ovarian cancer

目的探讨Lewisy抗原的表达与晚期卵巢癌患者化疗耐药的相关性。方法选择西北妇女儿童医院2014年6月~2018年12月期间收治的晚期卵巢癌患者67例,对所有患者Lewisy抗原的表达情况进行检测,根据患者对化疗的敏感程度分为耐药组和敏感组,对比两组患者的Lewisy抗原的表达情况和相关临床资料,并对Lewisy抗原表达情况与晚期卵巢癌患者化疗耐药的相关性进行分析,并探讨影响患者耐药的相关危险因素。结果本研究中39例患者为敏感组,28例患者为耐药组,耐药组患者Lewisy抗原表达阳性率、抗原表达表达强度均显著高于敏感组,同时耐药组Lewisy抗原的平均光密度值也显著高于敏感组,差异具有统计学意义(P <0.05);同时两组晚期卵巢癌患者的国际妇产科协会(FIGO)分期、组织分化程度、残余病灶大小及Lewisy抗原表达情况间差异有统计学意义(P <0.05);经过相关性分析后得到,晚期卵巢癌患者Lewisy抗原表达水平与患者的耐药情况呈正线性相关,(r=0.755,P <0.05);同时经过logistic多因素回归分析后得到FIGO分期Ⅳ期、Lewisy抗原阳性是影响晚期卵巢癌患者耐药的独立危险因素,差异具有统计学意义(P <0.05)。结论 Lewisy抗原的表达与晚期卵巢癌患者化疗耐药的发生密切相关,对于临床上评估患者的疗效和预后具有重要意义。

Objective To investigate the correlation between the expression of Lewis antigen and chemoresistance in patients with advanced ovarian cancer.Methods 67 patients with advanced ovarian cancer admitted to our hospital from June 2014 to December 2018 were selected to detect the expression of Lewis antigen.All patients were divided into drug-resistant group and sensitive group.The expression of Lewis antigen and related clinical data of the two groups were compared,and the correlation between the expression of Lewis antigen and chemotherapy resistance of advanced ovarian cancer patients was analyzed,And to explore the risk factors of drug resistance.Results In this study,39 patients were in the sensitive group and 28 patients were in the resistant group.The positive rate and intensity of expression of Lewis antigen in the resistant group were significantly higher than those in the sensitive group.Meanwhile,the average optical density of Lewis antigen in the resistant group was significantly higher than that in the sensitive group(P<0.05).At the same time,FIGO stage,FIGO stage and There were statistically significant differences in the degree of tissue differentiation,the size of residual lesions and the expression of Lewis antigen(P<0.05).After correlation analysis,it was found that the expression of Lewis antigen in patients with advanced ovarian cancer was positively correlated with the drug resistance(r=0.755,P<0.05);at the same time,after logistic analysis,the expression of Lewis antigen in patients with advanced ovarian cancer was positively correlated with the drug resistance(r=0.755,P<0.05)After multivariate regression analysis,FIGO stage IV and Lewis antigen positive were independent risk factors for drug resistance in patients with advanced ovarian cancer,the difference was statistically significant(P<0.05).Conclusion The expression of Lewisy antigen is closely related to the occurrence of chemotherapy resistance in patients with advanced ovarian cancer,which is of great significance for clinical evaluation of patients’efficacy and prognosis.
