来曲唑治疗多囊卵巢综合征临床研究

Letrozole in the Treatment of Polycystic Ovarian Syndrome

目的探讨来曲唑治疗多囊卵巢综合征(PCOS)的疗效及对患者内脂素、脂联素等激素水平的影响。方法选取医院门诊2017年3月至2019年3月收治的PCOS患者106例,按奇偶数法分为观察组和对照组,各53例。对照组患者给予枸橼酸氯米芬胶囊口服,观察组患者给予来曲唑片口服,两组患者均连续服药3个月。结果观察组总有效率为88.68%,明显高于对照组的71.70%(P<0.05);治疗后,观察组卵泡数明显少于对照组,子宫内膜明显厚于对照组,血清睾酮、黄体生成素、雌二醇、内脂素水平及卵巢过度刺激综合征发生率均明显低于对照组,脂联素水平及排卵率、妊娠率均明显高于对照组(P<0.05);观察组不良反应发生率为16.98%,与对照组的11.32%相当(P>0.05)。结论来曲唑治疗PCOS,可改善患者的相关临床症状,其作用机制可能与降低内脂素水平和升高脂联素水平有关。

Objective To observe the efficacy of letrozole in the treatment of polycystic ovary syndrome(PCOS)and to explore the mechanism of the expression of visfatin and adiponectin in patients with PCOS.Methods A total of 106 cases of PCOS patients admitted to the hospital from March 2017 to March 2019 were selected and divided into the observation group and control group according to the odd-even method,53 cases in each group.The control group was treated with clomiphene capsules,and the observation group was treated with letrozole tablets.Both groups were continuously treated for 3 months.Results The total clinical effective rate was 88.68%in the observation group,which was significantly higher than 71.70%in the control group(P<0.05).After treatment,the number of follicles in the observation group was significantly less than that in the control group,and the thickness of endometrium was significantly greater than that in the control group;the serum levels of testosterone,luteinizing hormone,estradiol and visfatin in the observation group were significantly lower than those in the control group,the levels of adiponectin,the ovulation rate and pregnancy rate were significantly higher than those in the control group(P<0.05).The incidence of adverse reactions was 16.98%in the observation group,which was comparable to 11.32%in the control group(P>0.05).Conclusion Letrozole is effective in the treatment of PCOS.It can improve the clinical symptoms of PCOS,and its mechanism is related to lowering the level of visfatin and increasing the level of adiponectin.
