摘要 目的探讨不同采集量和不同的制备时间对悬浮红细胞质量的影响,为不足量血制备的悬浮红细胞的应用提供参考。方法按照全血采血量和制备前全血不同保存时间作为标本分组依据将标本分为4个不足量血实验组和2个足量采血对照组,每组10袋,制备成悬浮红细胞。在制备d0、d7、d14、d21、d28、d35分别取样,检测红细胞计数及形态指标、常规质量控制指标和红细胞代谢及功能指标。结果与d0相比,各组标本各储存时间点的红细胞计数及形态指标变化均无统计学意义(P>0.05)。4个实验组与对照组相比,容量、Hct和Hb 3个指标差异均无统计学意义(P>0.05)。S2组储存期末溶血率高于C1组(P<0.05);S4组储存期末溶血率高于C2组(P<0.05)。4个实验组ATP、Glucose、2,3-DPG各个储存期差异均无统计学意义(P>0.05)。S2组LDH和LA储存d28起显著高于C1组(P<0.05)。S4组LDH、LA从储存d21起显著高于C2组(P<0.05)。S3组LDH、LA从储存d35显著高于C2组(P<0.05)。结论当全血采集量在50%—65%标识量时,在全血储存8 h内制备成悬浮红细胞,并在储存21 d内使用;全血采集量在66%—89%标识量时,可在全血储存8h内或8h后制备成悬浮红细胞,并在储存28 d内使用。 Objective To investigate the effect of collection volume and holding time on the quality of suspended RBC(sRBC)prepared from under-collection whole blood(UC-WB),so as to assess the safety and efficacy of sRBC prepared with UC-WB.Methods According to the volumes of whole blood collected and storage times,the whole blood samples were divided into 4 experimental groups,that is under-collection(UC)at 66%-89%of the labeled collection within and over 8 hours after collection(referred as group S1 and group S3,respectively),UC at 50%-65%of the labeled collection within and over 8 hours after collection(referred as group S2 and group S4,respectively),and 2 control groups[normal collection within and over 8 hours after collection(referred as group C1 and group C2,respectively)].Each groups had 10 bags.The properties of sRBC prepared from experimental groups,including red cell count,morphology,volume,Hct,Hb,hemolysis,ATP,Glucose and 2,3-DPG etc,at day 0,7,1421,28 and 35 after preparation,were analyzed and compared to control groups.Results During storage for 35 days,differences in red cell count and morphology in each group between d0 and d7/14/21/28/35 were not statistically significant(P>0.05).Differences between volume,Hct and Hb in 4 experimental groups and 2 control groups were not statistically significant(P>0.05).The incidence of hemolysis at the end of storage in group S2,S4 was higher than that in group C1,C2,respectively(P<0.05).During storage for 35 days,difference in ATP,Glucose and 2,3-DPG at each holding time between 4 experimental groups were not statistically significant(P>0.05).The differences between LDH and LA in group S2,S4,and S3 were significantly higher than group C1,C2 and C2 since d28,d21 and d35,respectively(P<0.05).Conclusion These results suggest that it is safe to transfuse sRBC prepared with UC-WB at 50%-65%volume of the labeled collection within 8 h after collection,and stored within 21 days.For UC-WB at 66%-89%volume of the labeled collection,sRBC can be prepared either within or over 8 h after collection,and be transfused within 28-day storage.
