深浅双骨道钢丝环扎联合克氏针固定治疗髌骨下极骨折的效果

Effect of deep and shallow double channel wire encircling and Kirschner wire fixation on the treatment of the fracture of the inferior patella

目的探讨深浅双骨道钢丝环扎联合克氏针的固定疗法对髌骨下极骨折的效果。方法选择2015年1月~2019年1月在海南医学院第一附属医院进行手术治疗的髌骨下极骨折患者80例为研究对象,按信封随机抽样分为对照组(克氏针钢丝张力带固定)和观察组(对照组基础上联合深浅双骨道钢丝环扎固定治疗),各40例。对两组患者的手术进行情况、术后并发症发生率、手术效果、随访结果、生活质量评分等进行统计分析。结果观察组手术时间、术中出血量、切口长度、C型臂X光机使用次数、手术并发症总发生率、开始下地活动时间、骨折完全愈合时间、视觉模拟评分均低于对照组(均P <0.05)。观察组手术优良率、生活质量各项评分均高于对照组(均P <0.05)。结论深浅双骨道钢丝环扎联合克氏针的固定疗法治疗髌骨下极骨折的效果显著,能够改善患者的手术效果,且手术时间短、创伤小、恢复快,膝关节功能改善满意,提高患者的术后生活质量。

Objective To study the effect of deep and shallow double bone channel wire encircling and Kirschner wire fixation on the fracture of the inferior patella.Methods From January 2015 to January 2019,80 patients with infrapatellar fracture who were operated in the First Affiliated Hospital of Hainan Medical College were randomly divided into control group(Kirschner wire tension band fixation),observation group(based on the control group combined with deep and shallow double bone channel wire encircling fixation treatment),40 cases in each group.The operation conditions,postoperative complication rate,operation effect,follow-up results and quality of life scores of the two groups were statistically analyzed.Results The operation time,intraoperative hemorrhage,incision length,the times of using C-arm X-ray machine,the total incidence of surgical complications,the time of starting to move to the ground,the time of complete fracture healing and the visual analogue score in the observation group were lower than those in the control group(all P<0.05).The excellent rate of operation and the scores of quality of life in the observation group were higher than those in the control group(all P<0.05).Conclusion The combination of deep and shallow double bone channel wire encircling and Kirschner wire fixation has a significant effect on the treatment of the fracture of the lower pole of patella,which can improve the surgical effect of the patients.The operation time is short,the trauma is small,the recovery is fast,the function of the knee joint is improved satisfactorily,and the quality of life of the patients after operation is improved.
