DRG评价方法在综合医院学科能力建设中的应用研究

Research on the Application of DRG Evaluation Method in the Discipline Capacity Building of General Hospital

目的:运用疾病诊断相关分组(DRG)对河南省某医院心血管内科学科能力进行分析,为医院指导学科建设,促进医院学科能力和诊疗水平提供参考。方法:基于某公司提供的CN-DRG分组方案,采用DRG相关指标分别对心血管内科整体情况、收治病组结构、科室绩效横向比较等方面进行分析。结果:心血管内科收治范围较广、专科特色不明显。部分重点病组时间、费用消耗指数较高,亟需采取控制措施。不同科室之间医疗服务绩效存在差异。结论:DRG评价能有效地对医院学科建设进行评估,明确学科发展的优劣势,便于医院调整病组结构,有针对性采取措施,控制某病组的时间、费用,提升学科能力水平和效益,促进医院精细化管理。

Objective:To analyze the disciplinary capacity of cardiology in a general hospital of He'nan Province based on DRG,it provides guidance for hospitals to improve disciplines construction and promote the disciplinary capacity.Methods:The CN-DRG grouping scheme was used to analysis the overall situation,the structure of disease groups,and the horizontal comparison of department performance of cardiology.Results:The scope of cardiovascular medicine treatment was wide and the specialties were not obvious.It is urgent to take measures to control disease groups whose time and cost consumption indices are high.There are differences in medical service capabilities between different departments.Conclusion:DRG can be used to effectively evaluate the disciplines construction of hospital,further to define the advantages and disadvantages of discipline development.The hospital can adjust the structure of the disease groups,take targeted measures to improve the level of competence and effectiveness of the discipline,and promote the refined management of the hospital.
