“I PASS ABCDE F”标准化交接班模式的构建与应用

Construction and Application of Standardized Shifting Mode of"I PASS ABCDE F"

目的:构建“I PASS ABCDE F”标准化交接班模式,结合护理信息化技术,降低ICU护士床旁交接班缺陷率,改善交接班环境,实现医护、护患、护护间无缝式沟通,达到保证患者安全的目的。方法:在ICU内实施标准化交接班模式,制作护士交班查检表、交接班电子指引单及电子交接模板,开展情景模拟案例+微课+系统培训课程的护士交接班培训等措施,评价开展前后的交接班效果。结果:护士交接班缺陷率降低至4.08%,医生对医护一体化查房及交接班时护士的满意度为(45.52±2.58)分(满分50分),护士之间的交接班满意度为(84.57±4.16)分(满分91分)。结论:应用“I PASS ABCDE F”标准化交接班模式,通过辅助电子信息表格及规范化培训,能改善交接班环境,减少护士交接班缺陷率,提高交接班质量,促进医护患沟通,保证患者安全。

Objective:To construct the standardized shifting mode of"I PASS ABCDE F",combine with the nursing information technology,reduce the defect rate of bedside shifting of ICU nurses,improve the shifting environment,realize the seamless communication between doctors and nurses,nurses and patients and nurses and nurses,and ensure the safety of patients.Methods:The standardized shifting mode is implemented in ICU.The shifting checklist for nurses,electronic guidance sheet of shifting and electronic shifting template are made.The measures are taken,such as shifting training for nurses by scene simulation case,WeChat class and system training course,and the shifting effected before and after it are evaluated.Results:The defect rate of shifting of nurses is reduced to 4.08%.The satisfaction of doctors to the integrated ward round and shifting of doctors and nurses and to nurses during shifting is(45.52±2.58)points(The full mark is 50 points),and the satisfaction of shifting between nurses is(84.57±4.16)points(The full mark is 91 points).Conclusion:Through the auxiliary electronic information form and standardized training,the application of the standardized shifting mode of"I PASS ABCDE F"can improve the shifting environment,reduce the defect rate of nurse shifting,improve the quality of shifting,promote the communication between doctors and patients,and ensure the safety of patients.
