摘要
【目的】探讨磁共振成像(MRI)不同扫描序列对闭合性跟腱撕裂的诊断价值.【方法】选取2016年1月至2019年1月本院收治的150例疑似闭合性跟腱撕裂患者,所有患者均于本院行MRI检查.根据MRI扫描序列的不同,分析比较T2WI+T1WI+SPAIR、T2WI+T1WI+STIR不同的扫描序列对闭合性跟腱撕裂的诊断价值.【结果】150例患者中,最终确诊闭合性跟腱撕裂96例.T1WI+T2WI+STIR序列诊断跟腱撕裂的灵敏性、特异性、准确性、阳性预测值和阴性预测值分别为91.67%、66.67%、82.67%、83.02%和81.82%;T1WI+T2WI+STIR序列诊断跟腱撕裂的灵敏性、特异性、准确性、阳性预测值和阴性预测值分别为95.83%、85.19%、92.00%、92.00%和92.00.T1WI+T2WI+STIR序列和T1WI+T2WI+SPAIR序列诊断跟腱撕裂的灵敏性、特异性、阳性预测值和阴性预测值比较,差异无统计学意义(P>0.05);T1WI+T2WI+SPAIR序列诊断跟腱撕裂的准确性为92.00%,明显高于T1WI+T2WI+STIR序列的81.82%,差异有统计学意义(P<0.05).【结论】MRI在闭合性跟腱撕裂诊断中有较好的应用价值,其中T1WI+T2WI+SPAIR序列诊断跟腱撕裂的准确性更高,值得临床参考.
【Objective】To investigate the diagnostic value of different MRI sequences in closed Achilles tendon tears.【Methods】From January 2016 to January 2019,150 patients with suspected closed rupture of Achilles tendon were selected.All patients were examined with MRI in our hospital.According to the different MRI sequences,the diagnostic value of T2 WI+T1 WI+SPAIR、T2 WI+T1 WI+STIR in closed Achilles tendon tears was analyzed and compared.【Results】Of the 150 patients,96 were diagnosed as closed Achilles tendon tear.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of T1 WI+T2 WI+STI were 91.67%,66.67%,82.67%,83.02%and 81.82%respectively;the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of T1 WI+T2 WI+STIR were 95.83%,85.19%,92.00%,92.00%and 92.00%respectively.There was no significant difference in sensitivity,specificity,positive predictive value and negative predictive value between T1 WI+T2 WI+STIR and T1 WI+T2 WI+SPAIR(P>0.05);the accuracy of T1 WI+T2 WI+SPAIR was 92.00%,significantly higher than 81.82%of T1 WI+T2 WI+STIR(P<0.05).【Conclusion】MRI has a good application value in the diagnosis of closed Achilles tendon tears,among which T1 WI+T2 WI+SPAIR sequence has a higher accuracy in the diagnosis of Achilles tendon tears,which is worthy of clinical reference.
