中药热奄包对下肢静脉性溃疡的促愈作用观察

Observation on promoting healing effect of traditional Chinese medicine foment package in treating venous ulcer of chronic lower extremity

[目的]通过观察中药热奄包治疗下肢静脉性溃疡的临床疗效,为临床治疗提供新的思路与方法。[方法]将90例下肢静脉性溃疡患者按随机数字表法随机分成两组,治疗组与对照组各45例。两组均给予重组牛碱性成纤维细胞生长因子凝胶外用作为基础治疗,治疗组在基础治疗之上给予中药热奄包穴位热敷治疗。比较两组患者治疗前、治疗后28 d临床疗效、溃疡面面积、愈合时间、症状体征积分,并进行不良反应和安全性分析的评价。[结果] 1)治疗组有效率为90.48%,对照组有效率为81.82%,治疗组临床疗效优于对照组,差异具有统计学意义(P<0.05)。2)两组症状体征积分、溃疡面面积均下降,治疗组下降更为明显,差异具有统计学意义(P<0.05)。3)治疗组愈合时间短于对照组,差异具有统计学意义(P<0.01)。4)两组基础治疗未见不良反应发生,中药热奄包穴位热敷不良反应未经特殊处理自行好转,两种疗法均比较安全。[结论]中药热奄包治疗下肢静脉性溃疡临床疗效良好,操作简便,不良反应少,安全性高,值得临床进一步研究和应用。

[Objective]To observe the clinical effect of traditional Chinese medicine(TCM)foment package in the treatment of venous ulcer of chronic lower extremity,and to provide a new method for clinical treatment.[Methods]Ninety patients with venous ulcer of lower limbs were divided into two groups according to a random number table method,with 45 cases in the treatment group and 45 cases in the control group.Both groups were treated with recombinant bovine basic fibroblast growth factor gel for external use as basic therapy,while the treatment group was treated with hot compress at acupoints of traditional Chinese medicine foment package on the basis of basic therapy.The clinical efficacy,ulcer area,healing time,symptom and sign scores of the two groups of patients before and after 28 days treatment were compared,and the adverse reactions and safety analysis were evaluated.[Results]The effective rate of the treatment group was 90.48%,and that of the control group was 81.82%.The total effective rate of the treatment group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).The symptom and physical sign scores and ulcer area of the two groups decreased,and the decrease in the treatment group was more obvious,with statistically significant difference(P<0.05).The healing time of the treatment group was shorter than that of the control group,with significant statistical difference(P<0.01).No adverse reactions occurred in the basic treatment of the two groups.The adverse reactions of acupoint hot compress of traditional Chinese medicine foment package improved without special treatment.Both therapies are relatively safe.[Conclusion]Traditional Chinese medicine foment package in the treatment of venous ulcer of chronic lower extremity has good clinical effect,few adverse reactions,high safety and simple operation.It is worthy of further clinical study and application.
