奖赏在儿童末梢采血中缓解焦虑和疼痛效果的比较性研究

A comparative study of the effects of reward on relieving anxiety and pain in peripheral blood samples of children

目的通过奖励卡通贴纸,探讨奖赏在儿童末梢采血时对焦虑和疼痛的缓解作用。方法随机选取200例在门诊检验科采血的患儿,随机分为对照组和奖赏组。对照组采用常规采血流程,不采取任何干预手段;奖赏组在采血前被告知进行采血可被奖励卡通贴纸。观察者使用儿童焦虑和疼痛评估量表评估患儿采血前及采血焦虑水平,使用面部疼痛修订评估量表评估患儿采血疼痛水平。结果采血前,对照组的焦虑水平(2.24±1.12)与奖赏组的焦虑水平(2.43±1.18)比较差异无统计学意义(P>0.05)。奖赏组患儿采血焦虑水平(1.77±1.05)低于对照组患儿的焦虑水平(2.26±1.01)(P<0.05)。奖赏组采血疼痛水平(5.85±1.83)低于对照组采血疼痛水平(6.65±1.84)(P<0.05)。结论以卡通贴纸作为奖赏是儿童末梢采血时缓解焦虑和疼痛的有效方法。

Objective To explore the effects of rewards on pain and anxiety during children′s peripheral blood collection by rewarding cartoon stickers.Methods A total of 200 patients in pediatric department were randomly selected from the clinical laboratory and randomly divided into the control group and the reward group.The control group used the routine blood collection process without any intervention;the reward group was told before the blood collection that they would be rewarded with a cartoon sticker.The observer used the pediatric anxiety and pain assessment scale to assess the level of anxiety before and after blood collection,and the facial pain revision assessment scale to assess the level of pain in blood collection.Results Before blood collection,there was no significant difference in the anxiety level between the control group(2.24±1.12)and the reward group(2.43±1.18)(P>0.05).Procedural anxiety scores in the reward group(1.77±1.05)were lower than the control group(2.26±1.01)(P<0.05).Pain scores among children in the reward group(5.85±1.83)were lower than the control group(6.65±1.84)(P<0.05).Conclusion The cartoon sticker rewards may be a potential way to relief pain and anxiety.
