联合检测PCT、SAA、CRP、IL-6在早期诊断细菌性血流感染中的研究 被引量:8

摘要 目的探讨联合检测PCT、SAA、CRP、IL-6,在细菌性血流感染早期诊断中的价值。方法回顾性分析我院2018年1月至2020年1月收治疑似血流感染患者200例,以最终诊断为细菌性感染患者为阳性组,以非细菌性感染患者为阴性组,分析两组患者PCT、SAA、CRP、IL-6检测结果,并比较其特异性、敏感性、阴性预测值与阳性预测值。结果阳性组患者PCT、SAA、CRP、IL-6检测水平均高于阴性组,差异具有统计学意义(P<0.001)。PCT+SAA+CRP+IL-6联合检测的特异性、敏感性、阴性预测值与阳性预测值均高于单项检测(P<0.05)。结论采取联合检测PCT、SAA、CRP、IL-6的方法,对早期诊断细菌性血流感的特异性、敏感性、阴性预测值与阳性预测值均高于单项检测,因此可以为临床早期选择抗生素治疗提供参考依据。 Objective To explore the value of combined detection of PCT,SAA,CRP and IL-6 in the early diagnosis of bacterial bloodstream infection.Methods A retrospective analysis of 200 patients with suspected bloodstream infection admitted to our hospital from January 2018 to January 2020,with the final diagnosis of bacterial infection as the positive group and non-bacterial infection as the negative group,analyzed two groups Patients'PCT,SAA,CRP,IL-6 test results,and compare their specificity,sensitivity,negativepredictive value and positive predictive value.Results The detection level of PCT,SAA,CRP and IL-6 in the positive group was higher than that in the negative group,and the difference was statistically significant(P<0.001).The specificity,sensitivity,negativepredictive values and positive predictive values of the combined test of PCT+SAA+CRP+IL-6 were higher than those of single test(P<0.05).Conclusion The combined detection of PCT,SAA,CRP and IL-6 has higher specificity,sensitivity,negative predictive valuesand positive predictive values for early diagnosis of bacterial blood influenza than single detection,so antibiotics can be selected for early clinical Treatment provides a reference basis.
