阿帕替尼联合替吉奥治疗老年晚期胃癌的临床效果观察

Clinical Observation of Apatinib Combined with Tegafur in the Treatment of Elderly Patients with Advanced Gastric Cancer

目的探究阿帕替尼联合替吉奥治疗老年晚期胃癌的临床效果。方法选取我院2017年6月至2018年6月收治的48例Ⅲ-Ⅳ期老年胃癌患者作为研究对象。将所有患者随机均分成实验组、对照组,各24例。给予对照组患者替吉奥胶囊单药治疗,给予实验组患者阿帕替尼+替吉奥胶囊联合治疗。观察、对比两组患者的治疗效果及不良反应发生情况。结果实验组患者治疗有效率33.33%高于对照组的8.33%,实验组患者疾病控制率87.50%高于对照组的54.17%,P<0.05,差异有统计学意义。所有患者经检查不良反应程度均在Ⅰ~Ⅱ度。两组患者的白细胞减少、血尿、乏力、腹泻、手足综合征、口腔溃疡等不良反应发生率相比,差异不明显,P> 0.05,差异无统计学意义。实验组患者血小板减少、蛋白尿、恶心呕吐及高血压等不良反应发生率均比对照组高,P<0.05,差异有统计学意义。结论与替吉奥胶囊单药相比,给予老年胃癌患者阿帕替尼联合替吉奥胶囊治疗可有效提高疾病控制率及治疗率,且安全性与替吉奥胶囊单药治疗相比相差不多,因此值得临床大力推广及应用。

Objectives To explore the clinical effect of apatinib combined with tegafur in the treatment of advanced gastric cancer in the elderly.Methods Forty-four patients with stage III-IV elderly gastric cancer admitted to our hospital from June 2017 to June 2018 were selected as subjects.All patients were randomly divided into experimental group and control group,24 cases each.The control group was given a single drug treatment with Tiggio capsules,and the experimental group was given a combination of apatinib+tigeol capsules.The therapeutic effects and adverse reactions of the two groups of patients were observed and compared.Results The effective rate of treatment in the experimental group was 33.33%higher than 8.33%in the control group.The disease control rate of the experimental group was 87.50%higher than that of the control group(54.17%,P<0.05).The difference was statistically significant.The degree of adverse reactions in all patients was 1 to II degrees.There was no significant difference in the incidence of adverse reactions such as leukopenia,hematuria,fatigue,diarrhea,hand-foot syndrome,and oral ulcer in the two groups,P>0.05,and the difference was not statistically significant.The incidence of adverse reactions such as thrombocytopenia,proteinuria,nausea and vomiting and hypertension in the experimental group were higher than those in the control group,P<0.05,and the difference was statistically significant.Conclusion Compared with the single drug of Tiggio capsule,the treatment of albinib combined with tigeol capsule in elderly patients with gastric cancer can effectively improve the disease control rate and treatment rate,and the safety is similar to that of Teggio capsule monotherapy.Therefore,it is worthy of clinical promotion and application.
