体外循环、非体外循环下冠脉搭桥术对患者心肺功能的影响分析 被引量:2

Analysis of the Effect of Cardiopulmonary Bypass Under Extracorporeal Circulation and Non Extracorporeal Circulation on Cardiopulmonary Function

摘要 目的探讨体外循环、非体外循环下冠脉搭桥术(CABG)对患者心肺功能的影响。方法根据随机及自愿原则将新乡医学院第一附属医院2016年6月—2019年6月三年期间收录的80例CABG患者分成对照组与观察组,其中对照组38例,观察组42例。对照组行体外循环冠脉搭桥术(ONCABG),观察组行非体外循环冠脉搭桥术(OPCABG),将两组患者手术后的心肺功能差异进行对比分析。结果两组患者术后用力肺活量(FVC)、第1 s用力呼气容积(FEV1)、1 s用力呼气量(PEF1)都有一定程度降低,但对照组降低更明显(P<0.05);两组患者术后24 h右室做功指数(RVSWI),左室做功指数(LVSWI)左心室收缩压(LVSP)明显低于手术前,且术后对照组明显低于观察组(P<0.05),心脏指数(CI)手术后显著升高,观察组升高更明显(P<0.05)。结论 ONCABG与OPCABG对患者心肺功能都有一定的影响,但相比于ONCABG,OPCABG对患者的心肺功能影响相对较小,临床优势更为明显,更适合临床推广与应用。 Objective To investigate the effect of cardiopulmonary bypass(CABG)under extracorporeal ciruculation and non extracorporeal circulation on cardiopulmonary function. Methods According to the principle of randomization and voluntariness,80 cases of CABG in the hospital from June, 2016 to June, 2019 were divided into control group and observation group,including38 cases in the control group and 42 cases in the observation group. The difference of cardiopulmonary function between the two groups was analyzed. Results FVC, FEV1 and PEF1 were decreased in two groups,but the decrease was more significant in the control group(P<0.05). RVSWI, LVSWI and LVSP after 24 h of two groups were significantly lower than those before operation,and those in control group were significantly lower than observation group(P<0.05). Cardiac index(CI)was significantly higher in observation group(P<0.05). Conclusion Both ONCABG and OPCABG have a certain influence on the cardiopulmonary function of patients,but compared with ONCABG,OPCABG has a relatively small influence on the cardiopulmonary function of patients,and its clinical advantages are more obvious,which is more suitable for clinical promotion and application.
