鳞状细胞癌相关抗原、胃泌素释放肽前体、细胞角蛋白片段19和组织多肽特异性抗原联合检测在肺癌诊断中的应用

Application of combined detection of SCCA,ProGRP,CYFRA-19 and TPSA in the diagnosis of lung cancer

目的探讨鳞状细胞癌相关抗原(SCC)、胃泌素释放肽前体(ProGRP)、细胞角蛋白片段19(CYFRA21-1)和组织多肽特异性抗原(TPS)联合检测在肺癌诊断中的应用价值。方法选择2015年1月-2018年12月在台州市第一人民医院住院的96例肺癌患者、35例肺良性病变患者和30例健康体检者(对照组)为研究对象,采用化学发光法检测SCC、ProGRP、CYFRA21-1和TPS,组间比较采用t检验进行统计学分析。结果SCC、ProGRP、CYFRA21-1和TPS在肺癌组中的表达显著高于肺良性病变组和对照组(P<0.05);SCC、ProGRP、CYFRA21-1和TPS在Ⅲ期+Ⅳ期中的表达水平显著高于Ⅰ期+Ⅱ期,在Ⅰ期+Ⅱ期中的表达水平显著高于对照组,差异均有统计学意义(P<0.05);单独检测敏感度和准确度较低,4项联合检测敏感度达83.33%,准确度达87.58%。结论SCC、ProGRP、CYFRA21-1和TPS对肺癌的辅助诊断有一定的价值,联合检测能提高检出率。

Objective To investigate the application value of combined detection of squamous cell carcinoma associated antigen(SCCA),gastrin releasing peptide precursor(ProGRP),cytokeratin fragment 19(CYFRA-19)and tissue polypeptide specific antigen(TPSA)in the diagnosis of lung cancer.Methods A total of 96 patients with lung cancer,35 patients with benign lung disease and 30 healthy subjects(control group)were enrolled into this study from the First People's Hospital of Taizhou during January 2015-December 2016.The chemiluminescence method was used to test SCCAA,ProGRP,CYFRA-19 and TPSA,and the t-test was used for statistical analysis.Results The levels of SCCA,ProGRP,CYFRA-19 and TPSA in the lung cancer group were significantly higher than those in the benign lung lesion group and the control group(P<0.05).The levels of SCCA,ProGRP,CYFRA-19 and TPSA in the III+IV phase were significantly higher than those in the I+II phase,the levels in the I+II phase were significantly higher than that of the control group(P<0.05);the sensitivity and accuracy of the single detection were lower,the sensitivity of the four joint detections was 83.33%,and the accuracy was 87.58%.Conclusion SCCA,ProGRP,CYFRA-19 and TPSA have certain value in the auxiliary diagnosis of lung cancer.Combined detection can improve the detection rate.
