急性脑梗死评估中GE256排Revolution CT一站式全脑灌注与CT血管成像的应用及临床价值 被引量:10

摘要 目的探讨GE256排Revolution CT一站式全脑灌注与CT血管成像(CTA)在急性脑梗死评估中的应用及临床价值。方法选取2019年1月至6月就诊于本院的疑似脑梗死患者65例作为研究对象,所有患者均出现不同程度的脑梗死症状,选用GE256排Revolution CT一站式全脑灌注+CTA检查患者脑动脉狭窄和闭塞情况,评估病情严重程度,以全脑数字减影血管造影(DSA)检查结果为金标准,比较2种检查诊断的效果。结果 2种诊断方法诊断脑梗死准确率比较,差异无统计学意义(P>0.05);2种诊断方法梗死部位诊断准确率比较差异无统计学意义(P>0.05);通过全脑灌注+CTA测定梗死中心区、周边区域及健侧脑灌注参数值,结果显示,梗死中心区平均通过时间(MTT)和达到峰值所需时间(TTP)水平明显高于周边区和健侧(P<0.05);脑血容量(CBV)和脑血流量(CBF)水平明显低于周边区和健侧,差异有统计学意义(P<0.05)。结论 GE256排Revolution CT一站式全脑灌注与CTA应用于脑梗死诊断评估中具有较高的准确率和敏感度,具有操作简单、无创、经济实惠等优点,可作为脑梗死诊断的主要方法之一,值得推广。 Objective To investigate the application and clinical value of GE256 row Revolution CT one-stop whole brain perfusion and CTA in the evaluation of acute cerebral infarction. Methods A total of 65 patients with suspected cerebral infarction who were admitted to our hospital from January 2019 to June 2019 were enrolled. All patients had different degrees of cerebral infarction. To examine the cerebral artery stenosis and occlusion, and assess the severity of the disease by GE256 row Revolution CT one-stop whole brain perfusion combined with CTA. With digital subtraction angiography(DSA) as the gold standard to compare the actual effects of the two examinations.Results There was no significant difference in the accuracy of diagnosing cerebral infarction and in the diagnostic accuracy of infarction sites between the two diagnostic methods(P>0.05). The infarct center, peripheral area and contralateral cerebral perfusion parameters could be determined by GE 256 row Revolution CT one-stop whole brain perfusion combined with CTA,and the MTT and TTP levels in the infarct center were significantly higher than those in the peripheral and healthy areas(P<0.05);the CBV and CBF levels were significantly lower than the surrounding areas and thaton the healthy side, the difference in data was statistically significant(P<0.05). Conclusion GE256 row Revolution CT one-stop whole brain perfusion and CTA have high accuracy and sensitivity in the diagnosis and evaluation of cerebral infarction. It has the advantages of simple operation, non-invasive and economical, and can be used as the main method for diagnosis of cerebral infarction,which is worth promoting.
