改良HEART评分对急诊胸痛患者个体化治疗的指导作用研究

目的分析改良HEART评分对急诊胸痛患者个体化治疗的指导作用。方法选取以胸痛为主诉的患者112例为研究对象,分别应用传统HEART评分与改良HEART评分对心源性胸痛(A组)、非心源性胸痛(B组)、A组中急性心肌梗死(A1组)与心绞痛(A2组)进行病情分析,分析不同危险分层患者血糖、糖化血红蛋白、心血管不良事件发生率。结果A组改良HEART评分与传统HEART评分均较B组高,且A组改良HEART评分也高于传统HEART评分(P<0.05);A1组改良HEART评分与传统HEART评分均较A2组高,且A1组、A2组的改良HEART评分均高于传统HEART评分(P<0.05);改良HEART评分中,随危险分层增加,患者血糖、糖化血红蛋白、心血管不良事件发生率增加(P<0.05)。结论改良HEART评分可准确鉴别分析急诊胸痛患者病情,用于指导急诊胸痛个体化治疗有较高价值,值得在临床推广实践。

Objective To analyze guidance effects of modified HEART score on individual treatment for emergency patients with chest pain.Methods A total of 112 patients with chest pain were enrolled.The traditional HEART score and modified HEART score were performed to analyze cardiac chest pain(group A),non-cardiac chest pain(group B),acute myocardial infarction(group A1)and angina pectoris(group A2)in group A.The blood glucose,glycosylated hemoglobin and incidence of cardiovascular adverse events in patients with different risk levels were analyzed.Results The modified HEART score and the traditional HEART score of group A were higher than those of group B.And the modified HEART score was higher than traditional HEART score in group A(P<0.05).The modified HEART score and traditional HEART score of group A1 were higher than those of group A2.And the modified HEART score was highertraditional HEART scores in group A1 and group A2(P<0.05).In the modified HEART score,blood glucose,glycosylated hemoglobin and incidence ofcardiovascular adverse events increased with the increase of risk level(P<0.05).Conclusion The modified HEART score can accurately identify and analyze the condition of emergency patients with chest pain.It is of relatively higher value for guiding individual treatment of chest pain in emergency.It is worthy of clinical promotion and practice.
