摘要
目的探讨老年心肌梗死合并高血压患者二级预防用药依从性的影响因素及护理对策。方法选取2017年5月至2019年5月在我院接受治疗好转并出院的老年心肌梗死合并高血压患者128例为研究对象,依照他汀类药物用药依从性评估标准将老年患者分为依从性好组83例和依从性差组45例。单因素分析患者对二级预防用药依从性情况,采用多因素logistic回归分析确定危险因素。结果多因素logistic回归分析显示,年龄、口服药物种类、二级预防用药的知晓程度、高血压病程以及距离上次住院时间是影响老年患者二级预防用药依从性的独立危险因素(P<0.05)。结论老年心肌梗死合并高血压患者二级预防用药依从性偏低,且受年龄、口服药物种类、二级预防用药认知、高血压病程、离院时间等诸多因素影响,医护人员应针对不同情况采取相应护理措施。
Objective To explore the influencing factors and nursing countermeasures for compliance of secondary preventive medication in elderly patients with myocardial infarction combined with hypertension.Methods To select 128 cases of elderly patients with myocardial infarction complicated with hypertension who were treated in our hospital from May 2017 to May 2019 and discharged as the study subjects,and the elderly patients were divided 83 cases into good compliance groups according to the evaluation criteria of statin drug compliance and 45 cases into poor compliance group.Univariate analysis was used to analyze the patients'compliance with secondary preventive medications and multi-factor logistic regression analysis were used to determine risk factors.Results Multivariate logistic regression analysis showed that the age,types of oral drugs,the level of awareness of secondary preventive medications,the course of hypertension,and the length of time before the last hospitalization were the independent risk factors that affected the compliance of secondary preventive medications in elderly patients(P<0.05).Conclusion The elderly patients with myocardial infarction complicated with hypertension had low compliance with secondary prevention medication,and were affected by many factors such as the age,types of oral drugs,the cognition of secondary preventive medications,the course of hypertension,and time of hospital discharge,so medical staff should take corresponding nursing measures for different situations.
