宫颈分泌物联合宫颈超声检查对早产预测的临床价值

The Clinical Value of Cervical Secretion Combined with Cervical Ultrasound in the Prediction of Preterm Birth

目的探讨宫颈分泌物中胎儿纤维连接蛋白(fFN)联合宫颈超声检查指标宫颈管长度(CL)在早产预测中的临床价值。方法选取2018年1月至2019年6月在我院就诊的有早产高危因素及宫缩异常频繁的未足月单胎妊娠孕妇100例,根据宫颈分泌物fFN水平,将产妇分为宫颈分泌物fFN>50ng/L和≤50ng/L组,观察两组早产发生情况。结果宫颈分泌物fFN>50ng/L和≤50ng/L产妇年龄、孕周及初产妇比例差异比较无统计学意义(P>0.05);宫颈分泌物fFN>50ug/L产妇早产发生率为80.56%,明显高于fFN≤50ng/L产妇(P<0.05);宫颈分泌物fFN>50ng/L产妇CL为(28.60±4.44)mm,明显低于fFN≤50ng/L产妇(P<0.05);宫颈分泌物fFN与CL呈负相关(r=-0.340,P<0.05);fFN、CL及联合诊断预测早产的ROC曲线下面积分别为0.760、0.803和0.904,P<0.05。结论宫颈分泌物中fFN联合宫颈超声检查指标CL在早产预测中有较好的应用价值,值得临床使用。

Objective To explore the clinical value of fetal fibronectin(fFN)in cervical secretion combined with cervical ultrasound length of cervical canal(CL)in the prediction of premature delivery.Methods From January 2018 to June 2019,100 pregnant women with high risk factors of premature delivery and abnormal frequency of uterine contractions were selected,according to the level of cervical secretion fFN,the pregnant women were divided into cervical secretion fFN>50ng/L and≤50ng/L group,the occurrence of premature delivery was observed in the two groups.Results There were no significant difference in age,gestational age and proportion of primipara between women whose cervical secretionfFN>50ng/L and≤50ng/L group(P>0.05);The incidence of preterm delivery in women with cervical secretion of fFN>50ng/L was 80.56%,which was significantly higher than that in women with fFN≤50ng/L(P<0.05);The CL of women with cervical secretion of fFN>50ng/L was(28.60±4.44)mm,significantly lower than that of women with fFN≤50ng/L(P<0.05);There was a negative correlation between fFN and Cl(r=-0.340,P<0.05);The area under ROC curve for fFN,CL and combined diagnosis of preterm delivery was 0.760,0.803 and 0.904,respectively,P<0.05.Conclusion fFN in cervical secretions combined with cervical ultrasonic examination index CL has a good application value in the prediction of premature birth,which is worthy of clinical use.
