胃肠超声造影联合血清肿瘤标志物在胃隆起性病变诊断中的应用价值

Value of gastrointestinal contrast-enhanced ultrasound combined with serum tumor markers detection in the diagnosis of gastric eminence lesions

目的研究胃肠超声造影联合血清肿瘤标志物对胃隆起性病变的诊断价值。方法选取广州中医药大学附属宝安中医院2016年1月至2019年12月经病理确诊的118例胃隆起性病变患者进行研究,其中良性组90例,恶性组28例,所有患者均行胃肠超声造影及血清肿瘤标志物糖链抗原(CA72-4)、卵巢癌抗原(CA125)、癌胚抗原(CEA)、癌相关黏液抗原(CA50)、消化道癌抗原(CA19-9)检测,分析胃肠超声造影联合血清肿瘤标志物检测诊断胃隆起性病变的准确性、敏感性和特异性。结果胃肠超声造影诊断胃隆起性病变的准确性、敏感性、特异性分别为74.6%、82.1%、72.2%;血清肿瘤标志物检测诊断胃隆起性病变的准确性、敏感性、特异性平均值为65.3%、47.2%、70.9%;胃肠超声造影联合血清肿瘤标志物检测诊断胃隆起性病变的准确性、敏感性、特异性分别为88.1%、92.9%、86.7%,明显高于两种方法单独诊断,差异有统计学意义(P<0.05)。结论胃肠超声造影联合血清肿瘤标志物检测对早期胃隆起性病变的诊断有较高的临床应用价值。

Objective To study the diagnostic value of gastrointestinal contrast-enhanced ultrasound combined with serum tumor markers detection in the diagnosis of gastric eminence lesions. Methods A total of 118 patients confirmed as gastric eminence lesions by pathological diagnosis in Bao’an Traditional Chinese Medicine Hospital Affiliated to Guangzhou University of Chinese Medicine between January, 2016 and December, 2019 are selected, including 90 cases in benign group, 28 cases in malignant group. All patients were performed gastrointestinal contrast-enhanced ultrasound and serum tumor markers(carbohydrate antigen CA72-4, ovarian cancer antigen CA125, carcinoembryonic antigen CEA, cancer-related mucus antigen CA50, gastrointestinal cancer antigen CA19-9) test, and the accuracy, sensitivity and specificity of gastrointestinal contrast-enhanced ultrasound combined serum tumor markers detection in the diagnosis of gastric eminence lesions were analyzed. Results The accuracy, sensitivity, and specificity in the diagnosis of gastric eminence were 74.6%, 82.1%, and 72.2% for gastrointestinal contrast-enhanced ultrasound, and 65.3%, 47.2%, and70.9% for serum tumor markers, respectively, which were significantly lower than 88.1%, 92.9%, and 86.7% for gastrointestinal contrast-enhanced ultrasound combined with serum tumor markers(P<0.05). Conclusion Gastrointestinal contrast-enhanced ultrasound combined with serum tumor markers is of great clinical value in the diagnosis of early gastric eminence.
