以精神障碍为首发症状的脑肿瘤患者的临床特点及预后分析

Analyses of the clinical feature and prognosis of cerebral tumor patients with mental disorders as first symptom

目的分析以精神障碍为首发症状的脑肿瘤患者的临床特点,并探讨影响精神症状预后的因素。方法选取2009年7月~2018年12月我院神经外科收治的103例以精神障碍为首发症状的脑肿瘤患者,所有患者均行手术治疗,术后患者的精神症状在我院精神科门诊继续治疗。分析精神障碍的类型、肿瘤的部位、肿瘤的性质对精神症状预后的影响。结果103例脑肿瘤全切93例,全切率90.3%。精神障碍治愈68例,好转19例,无效16例。术前患者精神障碍的类型和肿瘤性质不同,精神症状的改善差异有统计学意义(P<0.05);术前患者肿瘤的部位不同,精神症状的改善差异无统计学意义(P>0.05)。结论以精神障碍为首发症状的脑肿瘤手术患者,手术切除肿瘤后患者精神症状大多数预后良好,术前患者精神障碍的类型和肿瘤的性质对判断患者精神症状的预后有一定的意义,肿瘤所在的部位不能作为判断患者精神症状预后的指标。

Objective To analyze the clinical feature of cerebral tumor patients with mental disorders as first symptom,and to investigate the factors affecting the prognosis of mental symptoms.Methods From July 2009 to December 2018,103 cerebral tumor patients with mental disorders as first symptom were selected from neurosurgery department of our hospital.All patients were treated surgically,and the mental symptoms of postoperative patients continued to be treated in psychiatric outpatient department of our hospital.The influence on the prognosis of mental symptoms by the type of mental disorders,the location of tumor and the nature of tumor was analyzed.Results Of 103 cerebral tumors,93 were totally resected,with a total resection rate of 90.3%.68 cases of mental disorders were cured,19 cases were improved,and 16 cases were ineffective.The types of mental disorders and tumor properties of patients at baseline were different,and the improvement of mental symptoms was statistically significant(P<0.05).There was no significant difference in the improvement of mental symptoms between patients with different tumor locations at baseline(P>0.05).Conclusion For cerebral tumor surgery patients with mental disorders as first symptom,most of the patients’mental symptoms have a good prognosis after surgical resection of the tumor.The type of mental disorders and the nature of the tumor at baseline have certain significance for judging the prognosis of the patients’mental symptoms.The location of the tumor cannot be used as an indicator for judging the prognosis of the patients’mental symptoms.
