新型冠状病毒肺炎患者淋巴细胞变化与急性肺损伤严重程度的关系

Relationship between changes of lymphocyte and severity of acute lung injury in patients with COVID⁃19

目的分析新型冠状病毒肺炎(COVID-19)患者淋巴细胞(LYM)计数与急性肺损伤严重程度的关系。方法选取2020年1-2月入住我院确诊为COVID-19患者47例,将其分为"普通组、重症组、危重组",统计各组患者入科时及住院期间LYM计数、中性粒细胞(NEU)百分比,并记录肺损伤评分、APACHEⅡ及SOFA评分。结果危重组和重症组肺损伤、SOFA及APACHEⅡ评分均显著高于普通组(均P <0.05),而危重组评分均高于重症组(P <0.05);与普通组相比,危重组和重症组入科及住院最低LYM计数均显著降低(P <0.05),而危重组住院最低LYM计数显著低于重症组(P <0.01);LYM计数降低幅度与肺损伤严重程度呈正相关(P=0.037)。结论 LYM计数及降幅可作为评估COVID-19患者急性肺损伤严重程度及预后指标。

Objective To analyze the relationship between lymphocyte(LYM)count and the severity of acute lung injury in patients with 2019 novel coronavirus pneumonia(COVID⁃19).Methods 47 patients with COVID⁃19 were selected in our hospital from January to February 2020,which were divided into the general group,severe group and critical group.LYM and neutrophil percentage(NE%)were counted at admission and during hospitalization,and the lung injury score,APACHEⅡand SOFA score were recorded.Results The lung injury,SOFA and APACHEⅡscores of critical and severe groups were significantly higher than normal group(P<0.05),all scores of critical group were higher than severe group(P<0.05).Compared with general group,critical and severe groups had lower minimum LYM counts in admission and hospitalization(P<0.05),while critical group had lower minimum LYM counts compared with severe group(P<0.01).The severity of acute lung injury was positively correlated with the range of LYM count(P=0.037).Conclusion LYM count and the range of LYM are important markers which can predict the severity of acute lung injury and prognosis in COVID⁃19 patients.
