血清CA125、VEGF及IL-6联合检测在子宫内膜异位症诊断中的价值

Application of combined detection of serum CA125,VEGF and IL-6 in the diagnosis of endometriosis

①目的探讨血清癌抗原125(CA125)、血管内皮生长因子(VEGF)及白细胞介素-6(IL-6)联合检测在子宫内膜异位症(EMT)诊断中的应用价值。②方法选取2018年10月~2019年10月于承德医学院附属医院妇科行手术治疗的97例患者为研究对象,术后诊断为EMT的55例为观察组,并依据美国生育学会(ASF)分期标准将其分为:Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期,Ⅰ~Ⅱ期为观察组A,Ⅲ~Ⅳ期为观察组B;将术后诊断为畸胎瘤或浆液性囊腺瘤的42例设为对照组。对两组患者血清中CA125、VEGF、IL-6的表达水平进行组间及组内比较;评估CA125、VEGF和IL-6单独及联合检测在EMT诊断中的价值。③结果①观察组血清中CA125、VEGF及IL-6的表达水平均明显高于对照组,差异有统计学意义(P<0.05);②观察组内B组患者血清中CA125、VEGF、IL-6的表达水平均高于A组,差异有统计学意义(P<0.05);③血清CA125、VEGF、IL-6的曲线下面积(AUC)分别为0.840、0.591、0.536,敏感性分别为72.73%、14.55%、7.28%,而将三者联合检测,其敏感性仅为3.54%。④结论 CA125可以作为诊断晚期EMT的一个有效的血清肿瘤标志物,而CA125、VEGF及IL-6联合检测,并没有显著的临床应用价值。

Objective:To explore the application of serum antigen 125(CA125),vascular endothelial growth factor(VEGF)and interleukin-6(IL-6)in the diagnosis of endometriosis value.Methods From October 2018 to October 2019,97 patients were treated with gynecological surgery at the Affiliated Hospital of Chengde Medical College.Among them,55 patients with postoperative diagnosis of endometriosis were observation group,and the observation group was based on American fertility.The learning(ASF)staging criteria are divided into:stage I,stage II,stage III,stage IV,stage I to II as observation group A,stage III to IV as observation group B,and postoperative diagnosis of teratoma or serous 42 cases of cystic adenoma were used as a control group.Compare the expression levels of CA125,VEGF and IL-6 in serum of observation group and control group.Compare the above indexes of observation group A and B.Evaluate CA125,VEGF and IL-6 alone and combined detection the value in the diagnosis of endometriosis.Results①The expression levels of CA125,VEGF and IL-6 in the serum of the observation group were significantly higher than those in the control group(P<0.05).②The serum levels of CA125,VEGF and IL-6 in the observation group B were observed.The expression level was higher than that of observation group A,and the difference was statistically significant(P<0.05).③The areas under the curve(AUC)of serum CA125,VEGF,and IL-6 were 0.840,0.591 and 0.536,respectively,and the sensitivities were 72.73%,14.55%and 7.28%,and the sensitivity of the three tests was only 3.54%.Conclusion CA125 can be used as an effective serum tumor marker for the diagnosis of advanced endometriosis,and combined detection of CA125,VEGF and IL-6 has no significant clinical value.
