不同年龄段心肌梗死患者心肌酶、肌钙蛋白及BNP表达水平及其临床意义 被引量:7

Expression level and clinical significance of myocardial enzyme,troponin and BNP in patients with myocardial infarction at different age groups

摘要 目的:探讨不同年龄段心肌梗死(AMI)患者心肌酶、肌钙蛋白及血清B型脑钠肽(BNP)的表达水平的差异性,对比其临床诊断意义。方法:选取医院确诊的200例心肌梗死患者,将其纳入观察组,另选取同期体检健康者200名,将其纳入健康对照组。两组均进行心肌酶中血清肌酸激酶-同工酶(CK-MB)、乳酸脱氢酶(LDH)、肌酸激酶(CK)、肌钙蛋白和BNP检测,对比两组检测结果的差异性,以及各指标与年龄的相关性。结果:观察组的血清CK-MB、LDH、CK、肌钙蛋白和BNP含量显著高于对照组,组间差异均有统计学意义(t=90.498,t=35.373,t=41.797,t=40.397,t=17.812;P<0.05);观察组的血清CK-MB、LDH、CK、肌钙蛋白和BNP的阳性率显著高于健康对照组,组间差异均有统计学意义(x^2=162.628,x^2=125.346,x^2=150.253,x^2=187.564,x^2=149.327;P<0.05);健康对照组的低龄段CKMB、CK、LDH、肌钙蛋白和BNP水平较高龄段低,但差异无统计学意义。观察组的高龄患者CK-MB、CK、BNP水平均显著高于低龄患者(t=2.653,t=2.753,t=2.827;P<0.05)。结论:揭示不同年龄段AMI患者心肌酶、肌钙蛋白及BNP表达水平的差异,能提高急性心肌梗死的检出率,提高诊断的正确率,同时也为患者治疗节省了宝贵时间。 Objective:To investigate the differences of expression levels of myocardial enzymes,troponin and B-type natriuretic peptide(BNP)in patients with myocardial infarction at different age groups,and to compare clinical significance of them.Methods:200 patients with myocardial infarction who were confirmed in hospital were divided into observation group,and 200 healthy persons who were confirmed by physical examination at the same period were divided into healthy control group.Both two groups underwent the detections of myocardial enzymes included serum creatine kinase isoenzyme(CK-MB),lactate dehydrogenase(LDH),creatine kinase(CK),troponin and BNP.The differences of the results between the two groups were compared,and the correlation of each index and age was further analyzed.Results:The contents of serum CK-MB,LDH,CK,troponin and BNP of observation group were significantly higher than those of control group(t=90.498,t=35.373,t=41.797,t=40.397,t=17.812,P<0.05).The positive rates of serum CK-MB,LDH,CK,troponin and BNP of observation group were significantly higher than those of healthy control group(x^2=162.628,x^2=125.346,x^2=150.253,x^2=187.564,x^2=149.327,P<0.05).In healthy control group,the contents of serum CK-MB,LDH,CK,troponin and BNP of patients with low ages were lower than those of patients with high ages,while the differences of them were no significant.In observation group,the contents of serum CK-MB,LDH,CK,troponin and BNP of patients with high ages were significantly higher than those of patients with low ages(t=2.653,t=2.753,t=2.827,P<0.05).Conclusion:This paper reveals the differences of the expressions levels of myocardial enzymes,troponin and BNP of patients with acute myocardial infarction(AMI)at different age groups,and it can increase the relevance ratio of AMI,and improve the diagnostic accuracy,and save precious time for the treatment of patients.
