摘要 目的探讨新生儿与成年人血栓弹力图(TEG)的异同,以及不同性别新生儿、早产儿与足月儿TEG的差别。方法应用TEG仪对75例新生儿(实验组)和75例成年人(对照组)检测并比较主要TEG指标:R、K、α、MA、G与CI,同时比较不同性别新生儿、早产儿与足月儿TEG指标差异。结果实验组与对照组比较:R:(5. 86±1. 51) VS(6. 89±0. 97);K:(2. 68±1. 64) VS(1. 89±0. 57);angle:(58. 04±9. 87) VS(64. 26±5. 39);MA:(50. 37±7. 94) VS(61. 48±6. 58);G:(5. 38±2. 07) VS(7. 81±2. 07);CI:(-2. 06±2. 45) VS(-0. 56±1. 41),均P <0. 05。新生儿男性与女性、早产儿与足月儿TEG指标比较,均无显著差异(P> 0. 05)。结论新生儿凝血因子活性高于成人,纤维蛋白原功能、血小板功能、血凝块功能及CI低于成人。不同性别新生儿、早产儿与足月儿的凝血功能无差异。 Objective To compare the differences in thrombelastography(TEG)between newborn infants and adults,between male and female newborn infants,and between premature infants and full-term infants.Methods 75 newborn infants were enrolled in treatment group and 75 adults were enrolled in control group;the TEG indices of coagulation reaction time(R),kinetics of clot development(K),coagulation rate(αangle),maximum amplitude(MA),clot firmness(G)and Coagulation Index(CI)were detected and compared between cases in treatment group and in control group,between male and female newborn infants and between premature infants and full-term infants.Results The TEG indices between cases in treatment group and in control group were of statistical difference[R value:(5.86±1.51)vs.(6.89±0.97),K value:(2.68±1.64)vs.(1.89±0.57),αangle:(58.04±9.87)vs.(64.26±5.39),MA value:(50.37±7.94)vs.(61.48±6.58),G value:(5.38±2.07)vs.(7.81±2.07),CI value:(-2.06±2.45)vs.(-0.56±1.41),all P<0.05];no significant difference was found in the TEG indices between male and female newborn infants and between premature infants and full-term infants(P>0.05).Conclusions The activity of coagulation factors in newborn infants is higher than that in adults while the functions of fibrinogen,platelet and coagulation are poorer and the level of CI is lower in newborn infants than those in adults;there is no difference in blood coagulation function between newborn infants of different sex and between premature infants and full-term infants.
