摘要 目的:探讨BIPAP呼吸机治疗睡眠呼吸暂停并肺心病呼吸衰竭的可行性。方法:选取2018年6月至2019年6月谷城县人民医院收治的睡眠呼吸暂停并肺心病呼吸衰竭患者73例作为研究对象,按照随机数字表法分为对照组(n=36)和观察组(n=37)。对照组施以常规综合治疗,观察组施以BIPAP呼吸机治疗,对患者治疗前及治疗1周后的最长呼吸暂停时间、最长低通气时间、呼吸紊乱指数、最高Sa O2、最低Sa O2、Pa O2、Pa CO2、Sp O2进行观察。结果:与治疗前比较,治疗1周后观察组的最长呼吸暂停时间、最长低通气时间明显缩短,呼吸紊乱指数明显降低,最高Sa O2、最低Sa O2、Pa O2、Sp O2明显提升,Pa CO2明显降低,与对照组比较,差异有统计学意义(P <0. 05)。结论:BIPAP呼吸机治疗睡眠呼吸暂停并肺心病呼吸衰竭具有较高的可行性。 Objective: To investigate the feasibility of BIPAP ventilator in the treatment of sleep apnea and pulmonary heart disease respiratory failure. Methods: A total of 73 patients with sleep apnea and pulmonary heart disease who were admitted to our hospital from June 2018 to June 2019 were enrolled in the study. They were randomly divided into control group( n = 36) and observation group( n = 37). The control group was treated with routine comprehensive treatment,and the observation group was treated with BiPAP ventilator. The longest apnea time,the longest hypoventilation time,the respiratory disorder index,the highest SaO2,the lowest SaO2,PaO2,PaCO2 and SpO2 were observed before and after treatment. Results: Compared with before treatment,the longest apnea time and the longest hypoventilation time were significantly shortened,and the respiratory disorder index was significantly reduced. The highest SaO2,the lowest SaO2,PaO2,and SpO2 were significantly increased,and the PaCO2 was significantly decreased. The difference was significant compared with the control group( P < 0. 05). Conclusion: BIPAP ventilator has high feasibility in the treatment of sleep apnea and pulmonary heart disease respiratory failure.
