超早期神经内镜辅助小骨窗微创颅内血肿清除术治疗高血压性基底节区出血的效果 被引量:5

Effect of ultra-early neuroendoscopic assisted minimally invasive removal of intracranial hematoma through small bone window on hypertensive basal ganglia hemorrhage

摘要 目的探讨超早期神经内镜辅助小骨窗微创颅内血肿清除术治疗高血压性基底节区出血的效果。方法将我院收治的170例高血压性基底节区出血患者根据手术方式不同分为对照组和观察组,各85例。对照组给予超早期小骨窗微创颅内血肿清除术,观察组给予超早期神经内镜辅助小骨窗微创颅内血肿清除术。观察两组的治疗效果。结果观察组的手术时间短于对照组,血肿清除率高于对照组,术后并发症总发生率低于对照组(P<0.05)。术后1、3个月,两组患者NIHSS评分较术前明显降低,Fugl-Meyer评分较术前明显升高,且观察组优于对照组(P<0.05)。观察组术后6个月的各项生活质量评分及治疗总有效率高于对照组(P<0.05)。结论超早期神经内镜辅助小骨窗微创颅内血肿清除术治疗高血压性基底节区出血的效果显著,能缩短手术时间,提高血肿清除率,减少并发症,促进患者术后神经及运动功能恢复,提高其生活质量。 Objective To investigate the effect of ultra-early neuroendoscopic assisted minimally invasive removal of intracranial hematoma through small bone window on hypertensive basal ganglia hemorrhage.Methods One hundred and seventy patients with hypertensive basal ganglia hemorrhage admitted in our hospital were divided into control group and observation group according to different surgical methods,with 85 cases in each group.The control group was treated through ultra-early minimally invasive removal of intracranial hematoma through small bone window,while the observation group was treated with ultra-early neuroendoscopic assisted minimally invasive removal of intracranial hematoma through small bone window.The therapeutic effects of the two groups were observed.Results The operation time of the observation group was shorter than that of the control group,the clearance rate of hematoma was higher than that of the control group,and the total incidence of postoperative complications was lower than that of the control group(P<0.05).At 1 and 3 months after operation,NIHSS scores of the two groups were significantly lower than those before operation,Fugl-Meyer scores were significantly higher than those before operation,and those of the observation group were better than the control group(P<0.05).The quality of life scores at 6 months after operation and total effective rate of treatment in the observation group were higher than those in the control group(P<0.05).Conclusion Ultra-early neuroendoscopic assisted minimally invasive removal of intracranial hematoma through small bone window in the treatment of hypertensive basal ganglia hemorrhage has significant effect.It can shorten the operation time,improve the clearance rate of hematoma,reduce complications,promote the recovery of nerve and motor function,and improve the quality of life of patients.
