网络医院处方点评与反馈前后处方合理性分析

Analysis of the Reasonableness of Online Hospital Prescriptions Before and After Review and Feedback

目的探讨临床药师参与的网络医院处方点评与反馈对网络医院合理用药的影响,从而促进患者用药安全、合理。方法随机抽取本院网络医院2016年1月~2018年12月门诊处方3600张,依照《医院处方点评管理规范(试行)》《处方管理办法》《国家抗微生物治疗指南》《中成药临床应用指导原则》、药品说明书及相关文献,对其中的不规范处方、不适宜处方及超常处方进行回顾性点评,并针对不合理用药原因进行干预。结果通过临床药师对2016~2018年3600张处方的网络处方点评,用药品种数(由2.70种下降至1.96种)、抗菌药物使用率逐年下降,由43.77%下降至22.92%。处方的合理率不断提升,由72.08%上升至93.17%。结论临床药师开展网络处方点评,有助于实现用药合理化,具有重要的临床应用价值。

OBJECTIVE To explore the influence of online pharmacist prescription reviews and feedback on the rational use of medicines in online hospitals,so as to promote the safe and reasonable use of medicines in practice.METHODS A total of 3600 outpatient prescriptions in our hospital's network hospital from January 2016 to December 2018 were randomly selected.In accordance with the"Hospital Prescription Reviews Management Guidelines(Trial)"、"Prescription Management Measures"、"National Antimicrobial Treatment Guidelines"、"Chinese Guidelines for the clinical application of proprietary medicines",we retrospectively reviewed the irregular prescriptions,inappropriate prescriptions,and unusual prescriptions,and intervened for the reasons for unreasonable drug use.RESULTS Through clinical pharmacists'online prescription reviews,the number of medications(from 2.70 to 1.96),the use rate of antibacterials(from 43.77%to 22.92%),and the reasonable rate of prescriptions(from 72.08%to 93.17%)decreased year by year.CONCLUSION Online prescription review by clinical pharmacists is helpful to rationalize medication and has important clinical application value.
