从手法治疗思路、治疗原则、操作方法、操作要点等方面系统分析宋敏教授采用正骨理筋手法治疗椎动脉型颈椎病的临床经验。其认为,椎动脉型颈椎病的病机关键是颈项部的“骨错缝、筋出槽”,病机根本在于颈部筋骨平衡失调,强调正骨理筋手法治疗重在调衡筋骨、整复错缝、疏通经络、调畅气血,并提出“五个注重”的手法操作要点(即注重医患合作、整体施治、操作节奏、筋骨平衡、多症并调)。正骨理筋手法治疗椎动脉型颈椎病疗效确切,且操作简便、安全性高,值得临床借鉴。

This paper systematically analyses professor SONG Min’s clinical experience in using bone-setting and tendon-smoothing manipulation to treat cervical spondylosis of vertebral artery type(CSA)from the aspects of manipulation therapeutic ideas,therapeutic principles,manipulation methods,and manipulation key points etc.He holds that the key to the pathogenesis of CSA is"complete fracture and dystopia of tendon",and the basis of pathogenesis lies in imbalance in tendons and bones at the cervix,emphasizing that bone-setting and tendon-smoothing manipulation treatment focuses on regulating and balancing tendons and bones,reduction complete fracture,dredging meridians and activating qi and blood.And he proposes the manipulation key points with"five focuses"(namely focuses on doctor-patient cooperation,overall treatment,manipulation rhythm,tendon and bone balance,multi-symptom simultaneous adjustment).It has definite efficacy with simple convenient manipulation and high safety to use bone-setting and tendon-smoothing manipulation to treat CSA,which deserves clinical references.
