U型肛门镜下外痔切除内痔悬吊术治疗混合痔30例临床观察

Clinical observation on 30 cases of mixed hemorrhoids treated by external hemorrhoidectomy and internal hemorrhoid suspension under U-shaped anoscope

目的观察U型肛门镜下外痔切除内痔悬吊术治疗混合痔的临床疗效。方法将60例混合痔患者采用随机数字表法分为治疗组和对照组,各30例。对照组采用传统的外剥内扎术治疗,治疗组采用U型肛门镜下外痔切除内痔悬吊术治疗。比较2组的临床疗效、创面愈合时间及术后第1,3,9天视觉模拟评分法(VAS)评分。结果对照组总有效率为96.67%(29/30),治疗组为100.00%(30/30),2组比较差异无统计学意义(P>0.05)。2组术后第1天VAS评分比较差异无统计学意义(P>0.05);术后第3天、第9天,2组VAS评分均明显降低,与同组术后第1天比较差异均有统计学意义(P<0.05),且治疗组降低更明显,组间同期比较差异均有统计学意义(P<0.05)。治疗组创面愈合时间为(23.6±4.67)d,对照组为(28.00±5.38)d,2组比较差异有统计学意义(P<0.05)。结论U型肛门镜下外痔切除内痔悬吊术治疗混合痔临床疗效显著,可明显降低患者VAS评分,并缩短创面愈合时间,值得临床推广应用。

Objective To observe the clinical efficacy of mixed hemorrhoids treated by external hemorrhoidectomy and internal hemorrhoid suspension under U-shaped anoscope.Methods Sixty cases with mixed hemorrhoids were divided into treatment group and control group,with 30 cases in each.The control group was treated with traditional milligan-morgan hemorrhoidectomy,while the treatment group was treated with external hemorrhoidectomy and internal hemorrhoid suspension under U-shaped anoscope.The clinical efficacy,wound healing time and visual analogue scale(VAS)scores on the 1st,3rd and 9th day after operation of the 2 groups were compared.Results The total effective rate of the control group was 96.67%(29/30),that of the treatment group was 100.00%(30/30),and the difference was not statistically significant between the 2 groups(P>0.05).There was no significant difference in VAS scores between the two groups on the first day after operation(P>0.05);on the third and ninth day after operation,VAS scores of the two groups were significantly lower than that of the same group on the first day after operation(P<0.05),and the reduction in the treatment group was more significant,and the difference between the 2 groups in the same period was statistically significant(P<0.05).The wound healing time of the treatment group was(23.6±4.67)days,that of the control group was(28.00±5.38)days,and the difference between the 2 groups was statistically significant(P<0.05).Conclusion It has marked clinical efficacy,can markedly reduce VAS score of the patients and shorten wound healing time to use external hemorrhoidectomy and internal hemorrhoid suspension under U-shaped anoscope in the treatment of mixed hemorrhoids,which deserves clinical promotion and application.
