自拟益气养阴活血汤治疗气阴两虚型干眼疗效观察

The efficacy of self-made Yi-Qi-Yang-Yin Huoxue decoction in the treatment of patients with dry eye syndromes

目的观察自拟益气养阴活血汤治疗气阴两虚型干眼的疗效。方法62例(85眼)干眼症患者,按随机数表法分为研究组31例(44眼)和对照组31例(41眼),研究组采用自拟益气养阴活血汤治疗,对照组采用常规西药加针刺治疗。治疗1个月后评估疗效;于治疗前及治疗1个月后检查泪液分泌情况泪液分泌试验(SIt)、泪膜稳定性泪膜破裂时间(BUT)及角膜上皮细胞完整性角膜荧光素染色(FL);比较两组治疗前后干眼问卷评分及中医症状积分差异。结果研究组总有效率大于对照组(P<0.05)。治疗1个月后,两组SIt、BUT检查结果均较治疗前升高,且研究组高于对照组(P<0.05);FL评分、干眼问卷评分及中医症状积分均较治疗前降低,且研究组低于对照组(P<0.05)。结论自拟益气养阴活血汤联合常规西药治疗气阴两虚型干眼可显著改善患者患眼泪膜稳定性、泪液分泌情况及角膜上皮细胞完整性,减轻临床症状,疗效显著。

Objective To observe the efficacy of self-made Yi-Qi-Yang-Yin Huoxue decoction in the treatment of dry eye syndromes.Methods Sixty-two patients(85 eyes)with dry eye syndromes were divided into self-made Yi-Qi-Yang-Yin Huoxue decoction group(study group,n=31 with 44 eyes)and conventional western medicine treatment and acupuncture group(control group,n=31 with 41 eyes).The efficacy was evaluated after 1 month of treatment.The tear secretion examined by using Schirmer I test(SIt),tear film stability examined by using tear break-up time(BUT)and corneal epithelial cell integrity determined by using corneal fluorescein staining(FL)were performed before and after 1 month of treatment.The dry eye symptoms(dry eye questionnaire)and TCM symptoms scores were compared before and after treatment between the two groups.Results The total effective rate in the study group was greater than that in the control group(P<0.05).After 1 month of treatment,the results of SIt and BUT in the two groups were higher than those before treatment,and the indexes in the study group were higher than those in the control group(P<0.05).The FL score,dry eye questionnaire score and TCM symptoms scores were lower than those before treatment,and the scores in the study group were lower than those in the control group(P<0.05).Conclusion Self-made Yi-Qi-Yang-Yin Huoxue decoction combined with conventional western medicine for dry eye syndromes showing Qi-Yin deficiency can significantly improve the tear film stability,tear secretion and corneal epithelial cell integrity,and improve the clinical symptoms.It has significant efficacy.
