集束化护理模式对老年髋部脆性骨折患者临床疗效炎性因子及生活质量的影响

The effect of cluster nursing mode on the clinical efficacy,inflammatory factors and quality of life of the elderly patients with hip fragility fracture

目的探讨集束化护理模式对老年髋部脆性骨折患者临床疗效、炎性因子及生活质量的影响。方法将100例髋部脆性骨折术后患者按随机数字表法分为观察组和对照组,每组各50例。2组患者均给予鲑鱼降钙素鼻喷剂联合口服钙尔奇D治疗,对照组同时给予常规护理,观察组给予集束化护理模式,比较2组患者临床治疗效果、血清炎性因子水平及生活质量。结果干预后,观察组骨密度(BMD)值高于对照组(P<0.05),观察组总有效率为86%,对照组为68%,观察组总有效率高于对照组(P<0.05)。干预后,2组血清中肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6、IL-1β的含量较干预前显著降低(P<0.05),且观察组低于对照组(P<0.05)。干预后,观察组患者自己照顾自己的能力、日常生活能力及焦虑/抑郁均优于对照组(P<0.05)。干预后2组患者的五维健康量表视觉模拟评分量表(EQ-VAS)得分均高于干预前(P<0.05);观察组患者的EQ-VAS得分高于对照组(P<0.05)。结论集束化护理模式可提高老年髋部脆性骨折患者临床疗效,减轻炎性因子水平,提高患者生活质量。

Objective To explore the effect of cluster nursing mode on the clinical efficacy,inflammatory factors and quality of life of elderly patients with hip fragility fracture.Methods One hundred patients with fragility fracture of hip were randomly divided into observation group and control group,50 cases in each group.Both groups were treated with salmon calcitonin nasal spray combined with oral calq-d,while the control group was treated with routine nursing at the same time.The observation group was treated with cluster nursing mode,and the clinical treatment effect,serum inflammatory factor level and quality of life of the two groups were compared.Results After intervention,the BMD value of the observation group was higher than that of the control group(P<0.05),the total effective rate of the observation group was 86%,the control group was 68%,and the total effective rate of the observation group was higher than that of the control group(P<0.05).After intervention,the levels of TNF-α,IL-6 and IL-1βin the serum of the two groups were significantly lower than those before intervention(P<0.05),and the levels in the observation group were lower than those in the control group(P<0.05).After intervention,the ability of self-care,daily life and anxiety/depression in the observation group were better than those in the control group(P<0.05).After the intervention,the scores of EQ-VAS in the two groups were higher than those before the intervention(P<0.05);the scores of EQ-VAS in the observation group were higher than those in the control group(P<0.05).Conclusion The cluster nursing mode can improve the clinical effect,reduce the level of inflammatory factors and improve the quality of life of the elderly patients with hip fragility fracture.
