高场MRI检查与乳腺钼靶对乳腺癌的诊断价值对比 被引量:3

High field MRI and mammary gland molybdenum target value to the diagnosis of breast cancer

摘要 目的比较高场磁共振成像(MRI)与X线钼靶诊断乳腺癌的临床价值。方法回顾性分析2018年1月至2018年12月本院收治的90例乳腺肿瘤患者的临床资料,以粗针穿刺病理学活检结果为金标准,比较高场MRI与X线钼靶鉴别诊断乳腺肿瘤良恶性的准确度。结果高场MRI诊断乳腺癌的灵敏度为90.16%(55/61),特异度为65.52%(19/29),阳性预测值为84.62%(55/65),阴性预测值为76.00%(19/25),准确度为82.22%(74/90),Kappa值为0.578。X线钼靶诊断乳腺癌的灵敏度为81.97%(50/61)、特异度为41.38%(12/29),阳性预测值为74.63%(50/67),阴性预测值为52.17%(12/23),准确度为68.89%(62/90),Kappa值为0.267,准确度明显低于高场MRI(P<0.05)。结论高场MRI相较于X线钼靶能提供更准确的乳腺肿瘤诊断信息,对早期确诊乳腺癌有重要临床价值。 Objective To compare high field magnetic resonance imaging(MRI) and the clinical value of X-ray mammography in the diagnosis of breast cancer. Methods Clinical data of 90 cases of mammary cancer patients were retrospectively analyzed,with coarse needle biopsy pathology biopsy results as the gold standard, the high field MRI and X-ray mammography differential diagnosis accuracy of breast benign and malignant tumor. Results The sensitivity of high field MRI in the diagnosis of breast cancer was 90.16%(55/61), specificity was 65.52%(19/29), positive predictive value was 84.62%(55/65), negative predictive value was 76.00%(19/25), accuracy was 82.22%(74/90), Kappa value was 0.578. The sensitivity of X-ray mammography in the diagnosis of breast cancer was 81.97%(50/61), specificity was 41.38%(12/29), positive predictive value was74.63%(50/67), negative predictive value was 52.17%(12/23), accuracy was 68.89%(62/90), Kappa value was 0.267, significantly lower than the high field MRI accuracy(P<0.05). Conclusion High field MRI compared with X-ray mammography can provide more comprehensive information of breast tumor diagnosis, has important clinical value for early diagnosis of breast cancer.
