腹腔镜子宫肌瘤剔除术与传统开腹手术治疗子宫肌瘤的临床疗效对比

Comparison of clinical efficacy between laparoscopic myomectomy and traditional open surgery for uterine fibroids

目的探讨腹腔镜子宫肌瘤剔除术、传统开腹手术对子宫肌瘤的治疗价值及对患者生活质量的影响。方法选取2015年4月至2018年4月本院收治的98例子宫肌瘤患者,采用随机数字表法分为研究组与对照组,各49例。研究组行腹腔镜子宫肌瘤剔除术,对照组行传统开腹手术。比较两组手术情况、效果、近期并发症和远期预后。结果两组手术成功率均为100.00%,研究组手术中出血量、术后住院时间显著少于对照组(P<0.05),两组手术时间比较差异无统计学意义;研究组术后并发症发生率(6.12%)显著低于对照组(20.41%)(P<0.05);两组术前SF-36量表评分比较差异无统计学意义,随访6个月后,研究组SF-36量表评分上升幅度优于对照组(P<0.05);随访期间研究组疾病复发率(2.04%)低于对照组(14.29%)(P<0.05)。结论应用腹腔镜子宫肌瘤剔除术治疗子宫肌瘤患者有效性、安全性较高,可有利于保障其预后及生活质量。

Objective To investigate the value of laparoscopic myomectomy and traditional open surgery for the treatment of uterine fibroids and the impact on patients’ quality of life. Methods 98 patients with uterine fibroids admitted in our hospital from April 2015 to April 2018 were selected and divided into study group and control group by random number table method, with 49 cases in each group. The study group underwent laparoscopic myomectomy, and the control group underwent traditional laparotomy. The surgical conditions effects, short-term complications and longterm prognosis were compared between the two groups. Results The operation success rate of the two groups was 100.00%. The amount of bleeding during the operation and the length of hospital stay in the study group were significantly less than in the control group(P<0.05). The comparison of the operation time between the two groups was no statistically significant. Before operation, the incidence of postoperative complications in the study group(6.12%) was significantly lower than that in the control group(20.41%)(P<0.05). The preoperative SF-36 scale score between the two groups was not statistically significant difference. After 6 months of follow-up, the SF-36 scale score in the study group was better than the control group(P<0.05), the disease recurrence rate(2.04%) in the study group was lower than that in the control group(14.29%) during the follow-up period(P<0.05). Conclusion Laparoscopic myomectomy is effective and safe for patients with uterine fibroids, which is conducive to ensuring their prognosis and quality of life.
