休克指数对创伤患者病情判断的临床意义

Clinical significance of shock index in judging the condition of trauma patients

目的探讨休克指数(SI)对创伤患者病情判断的临床意义。方法选取2015年1月至2018年12月遵义市第一人民医院重症医学科收治的创伤患者126例,分析呼吸、心率、收缩压、舒张压、平均动脉压(MAP)、SI指标水平与损伤严重度评分(ISS)的相关性。结果呼吸水平与ISS评分呈弱正相关(r=0.259),心率水平与ISS评分呈强正相关(r=0.639),收缩压水平与ISS评分呈强负相关(r=-0.505),舒张压水平与ISS评分呈中等负相关(r=-0.308),MAP水平与ISS评分呈中等负相关(r=-0.408),SI水平与ISS评分呈强正相关(r=0.681)。结论SI比呼吸、心率及血压更能反映创伤病情的危重程度,对判断患者早期病情有一定临床价值,可予以推广,但老年、儿童等人群使用年龄调整SI可能更为恰当。

Objective To explore the clinical significance of shock index(SI)in judging the condition of trauma patients.Methods From January 2015 to December 2018,126 trauma patients in the department of critical medicine of the First People′s Hospital of Zunyi were selected.The correlation between the level of respiration,heart rate,systolic pressure,diastolic pressure,mean arterial pressure(MAP),SI and injury severity score(ISS)were analyzed.Results There was a weak positive correlation between respiratory level and ISS score(r=0.259).There was a strong positive correlation between heart rate level and ISS score(r=0.639).There was a strong negative correlation between systolic blood pressure level and ISS score(r=-0.505).There was a medium negative correlation between diastolic blood pressure level and ISS score(r=-0.308).There was a medium negative correlation between MAP level and ISS score(r=-0.408).There was a strong positive correlation between SI level and ISS score(r=0.681).Conclusion SI can reflect the severity of trauma more than breath,heart rate and blood pressure,and it has a certain clinical value in judging the early stage of patients′condition,which can be popularized,but it may be more appropriate for the elderly,children and other groups to use age adjustment SI.
