颅内肿瘤术后麻醉恢复期并发颅内血肿的影响因素分析及护理

Analysis and nursing of influencing factors of intracranial tumor after anesthesia recovery and intracranial hematoma

目的探究颅内肿瘤术后麻醉恢复期并发颅内血肿的影响因素,并以此制定相应的护理干预措施,改善患者病情。方法选取2014年1月—2019年1月在我院进行颅内肿瘤手术治疗的770例患者临床资料进行分析,探究其在颅内肿瘤术后麻醉恢复期并发颅内血肿的影响因素,并以此制定护理方案。结果 770例患者中,并发颅内血肿患者18例,发生几率为2.34%。单因素分析显示,肿瘤类型、术前伴有高血压、糖尿病、术中行颅内外引流术及术中出血量>800 ml,是麻醉恢复期并发血肿的影响因素;多因素Logistic回归分析结果显示,肿瘤类型、术中出血量大、术前伴有高血压为术后麻醉恢复期并发血肿的独立危险因素。结论颅内肿瘤术后麻醉恢复期并发颅内血肿的影响因素主要包括肿瘤类型,术前伴有高血压及糖尿病、术中行引流干预及术中出血量较大,因此,应通过加强对该类患者术后恢复期生命体征的监护,术前改善患者血压及血糖水平,并对高危患者进一步加大护理措施等方式,避免其在术后恢复期并发颅内出血。

Objective To investigate the influencing factors of intracranial tumor after anesthesia recovery and the intracranial hematoma,and to develop corresponding nursing interventions to improve the patient’s condition. Methods The clinical data of 770 patients who underwent intracranial tumor surgery in our hospital from January 2014 to January 2019 were analyzed to investigate the factors affecting intracranial hematoma during postoperative anesthesia recovery of intracranial tumors.Develop a care plan. Results hematoma,the incidence rate was 2.34%.Univariate analysis showed that the tumor type,preoperative hypertension,diabetes,intracranial and extracranial drainage during the operation,and intraoperative blood loss were> 800 ml.It is the influencing factor of hematoma in the recovery period of anesthesia.The results of multivariate logistic regression analysis showed that tumor type,high intraoperative blood loss,and hypertension before operation were independent risk factors of hematoma in the recovery period of anesthesia. Conclusion The main factors influencing the intracranial hematoma after intracranial tumor reconstruction include intratumoral type,preoperative hypertension and diabetes,intraoperative drainage and intraoperative blood loss.Therefore,this category should be strengthened.The patient’s monitoring of vital signs during the recovery period,improve the patient’s blood pressure and blood glucose levels before surgery,and further increase the nursing measures for high-risk patients to avoid intracranial hemorrhage during the recovery period.
