前列舒通胶囊辅助治疗良性前列腺增生患者经尿道前列腺电切术后疗效及影响因素分析

Analysis on the therapeutic effect and influencing factors of Qianlie Shutong capsule in adjuvant treatment patients with prostatic hyperplasia after transurethral resection of prostate

目的探讨前列舒通胶囊辅助治疗良性前列腺增生(BPH)患者经尿道前列腺电切术(TURP)术后的疗效,并分析相关因素对BPH患者TURP术后的影响。方法选取2016年3月—2019年4月就诊的BPH患者168例,随机分为观察组和对照组,每组84例。两组均予以TURP治疗,术后观察组服用前列舒通胶囊,疗程1个月。观察两组患者术前及术后1个月最大尿流率(Qmax)和残余尿量(RU),采用国际前列腺症状评分(IPSS)和生活质量指数(QOL)对两组患者进行评分,并记录两组术后1个月内的不良反应。采用单因素分析和多元Logistic回归模型分析BPH患者TURP术后病情的影响因素。结果治疗后,两组患者Qmax高于治疗前,RU、IPSS评分、QOL评分均低于治疗前(P<0.05),且观察组各指标改善程度均优于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。两组单因素和多因素分析显示,年龄、病程、术前IPSS评分、前列腺质地、服用前列舒通胶囊均与BPH患者术后病情有关(P<0.05),是BPH患者术后病情的独立影响因素(P<0.05)。结论BPH患者TURP术后服用前列舒通胶囊辅助治疗可提高Qmax,降低RU、IPSS评分、QOL评分,有效减轻前列腺症状,且患者TURP术后病情恢复情况与患者年龄、病程、术前IPSS评分和服用前列舒通胶囊均相关。

Objective To investigate therapeutic effect of Qianlie Shutong capsule in adjuvant treatment patients with benign prostatic hyperplasia(BPH)after transurethral resection of prostate(TURP),and analyze the influence of related factors on BPH patients after TURP.Methods Total of 168 patients with BPH were selected in the urology department of our hospital from March 2016 to April 2019,and divided into the observation group(n=84)and the control group(n=84)randomly.Both groups were treated with TURP.On this basis,the observation group was taken qianlie shutong capsule after TURP,course of treatment for 1 month.The control group received no other treatment.Before and 1 month after surgery,the maximum urine flow rate(Qmax)and residual urine volume(RU)of the two groups were observed,and the international prostate symptom score(IPSS)and quality of life index(QOL)were used to score the patients of the two groups,and the adverse reactions of the two groups within one month after surgery were recorded.Univariate analysis and multivariate Logistic regression model were used to analyze the influencing factors of patients with BPH after TURP.Results After treatment,the Qmax of the two groups was higher than before treatment,and the RU,the scores of IPSS and QOL were lower than before treatment(P<0.05).The observation group had higher Qmax than the control group,and the RU,the scores of IPSS and QOL were lower than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups during the treatment period(P>0.05).Univariate analysis showed that age,course of disease,preoperative IPSS score,prostate texture and the use of Qianlie Shutong capsule were all associated with the postoperative condition of patients with BPH(P<0.05).Multivariate analysis showed that age,course of disease,preoperative IPSS score and the use of propiastone capsules were independent influencing factors for postoperative outcomes of patients with BPH(P<0.05).Conclusion After TURP surgery,the BPH patients accepted adjuvant treatment of Qianlie Shutong capsule can improve Qmax,reduce RU,scores of IPSS and QOL,and effectively reduce the prostate symptoms.Patients′age,course of disease,preoperative IPSS score and the use of Qianlie Shutong capsule were all associated with the TURP in BPH patients.
