指扩法治疗小儿肛裂的临床疗效观察

The clinical effect of using a finger for anal dilatation on children with anal fissure

目的观察指扩法治疗小儿肛裂的临床疗效,为小儿肛裂的治疗提供安全有效的治疗方式。方法选择2017年1月—2019年12月肛肠科门诊就诊的小儿肛裂患者60例,将采用保守治疗效果欠佳但又不接受手术而采取指扩法治疗者归为治疗组(27例),愿意继续保守治疗者归为对照组(33例)。比较两组患儿临床疗效及排便时肛门疼痛、出血情况。结果治疗第15天后,治疗组患儿临床有效率为88.89%(24/27),对照组为57.58%(19/33),两组比较差异有统计学意义(P<0.05)。治疗第7、15天后,两组患儿的疼痛评分均低于治疗前(P<0.05),且治疗组患儿的疼痛评分明显低于对照组,差异有统计学意义(P<0.05)。治疗第15天后,两组患儿的便血评分均低于治疗前(P<0.05),且治疗组患儿的便血评分明显低于对照组,差异有统计学意义(P<0.05)。结论指扩法能有效改善患儿肛裂疼痛、便血症状,安全有效,操作简单,值得推广。

To observe the clinical effect of finger dilation in treating anal fissure in children,and to provide a safe and effective treatment for the treatment of anal fissure in children.Methods 60 cases of pediatric with anal fissure patients who were treated in the anorectal clinic of our hospital from January 2017 to December 2019 were selected.Those with conservative treatment but who did not accept surgery adopted the finger expansion method as the treatment group(27 cases),and those who were willing to continue conservative treatment were the control group(33 cases).The clinical efficacy and anal pain and bleeding during defecation were compared between the two groups.Results After the 15th day of treatment,the clinical effective rate of the children in the treatment group was 88.89%(24/27)and 57.58%(19/33)in the control group.The difference between the two groups was statistically significant(P<0.05).After 7 and 15 days of treatment,the pain scores of the two groups of patients were lower than before treatment(P<0.05),and the pain scores of the children in the treatment group were significantly lower than the control group,the difference was statistically significant(P<0.05).After the 15th day of treatment,the blood stool scores of the two groups of patients were lower than before treatment(P<0.05),and the blood stool scores of the children in the treatment group were significantly lower than the control group,the difference was statistically significant(P<0.05).Conclusion Finger dilation method can effectively improve the symptoms of anal fissure pain and bloody stool in children.It is safe and effective,and the operation is simple,which is worthy of promotion.
