胃肠胰神经内分泌肿瘤临床特点及预后

Clinical characteristics and prognosis of gastroenteropancreatic neuroendocrine neoplasms

目的:研究胃肠胰神经内分泌肿瘤的临床特点及预后。方法:共纳入89例胃肠胰神经内分泌肿瘤(GEP-NENs)患者,观察比较性别、年龄、发病部位、临床症状、病理分型、肿瘤分期等不同因素与预后方面的差异。结果:89例患者中男54例,女35例,平均发病年龄(57.50±11.03)岁,其中最常见好发部位为胃、胰腺、直肠。患者症状多样性,且与原发部位相关。89例GEP-NENs,G1期患者12例(13.48%),G2期25例(28.09%),G3期52例(58.42%)。发生淋巴结转移40例,远处转移12例。手术是其主要治疗方式,同时联合细胞毒性药物治疗和分子靶向药物治疗。单因素分析显示肿瘤分级G3、肿瘤浸润深度T4、发生淋巴结转移、发生远处转移为其相关风险因素。结论:GEP-NENs发病率逐年增加,临床表现无特异性,治疗以手术为主,预后与病理分型及肿瘤分期相关。

Objective: To investigate clinical characteristics and prognosis of gastroenteropancreatic neuroendocrine neoplasms. Methods: The gender,age,primary site,clinical features,pathological characteristics,tumor stage and prognosis of the 89 patients with GEP-NENs were summarized and analyzed. Results: There were 89 cases(54 males and 35 females) with a mean age of (57.50±11.03) years.The most common sites were stomach, pancreas and rectum. They have different clinical manifestations and were closely related to the primary site. Of all the patients,G1 phase 12 cases(13.48%), G2 phase 25 cases(28.09%) and G3 phase 52 cases(58.42%). There were 40 cases of lymph node metastasis and 12 cases of distant metastasis. Surgery was the main therapeutic approach, combined with cytotoxic drug therapy and molecular targeted drug therapy. Univariate analysis showed that tumor grade, invasion depth,lymph node metastasis and distant metastasis were poor prognostic factors.Conclusion: The incidence of GEP-NENS increased year by year, but the clinical manifestations are non-specific. Surgery is the mainly treatment and the prognosis is related to pathological classification and tumor stage.
