经皮内镜胃造瘘术管在基层医院开腹胃造瘘术中的应用

Application of PEG tube in open gastrostomy in basic hospital

目的:总结分析经皮内镜胃造瘘术管在开腹胃造瘘术中的应用技巧,开腹胃造瘘术的并发症发生情况,为提高手术成功率、减少术后并发症提供依据。方法:回顾性分析我院2015年1月1日至2019年7月1日行开腹胃造瘘术患者31例,探讨开腹胃造瘘术的手术技巧,术后并发症发生情况以及处理方法。结果:31例均顺利完成胃造瘘手术,其中3例术后出现造瘘口旁渗漏并皮炎,3例于1年左右出现脱管,1例造瘘管堵塞;1例造瘘管断裂,1例包埋综合征,1例局部肉芽组织增生。经及时处理均取得较好的临床效果。结论:开腹使用经皮内镜胃造瘘术管行胃造瘘术是一项简单、安全的手术方法,结合了内镜下胃造瘘术及开腹手术的部分优点,值得基层医院推广。

Objective:To summarize and analyze the application skills of PEG tube in open gastrostomy,the occurrence of complications of open gastrostomy,and provide a basis for improving the success rate of surgery and reducing postoperative complications.Methods:A retrospective analysis of 31 patients undergoing open gastrostomy from January 1,2015 to July 1,2019 in the People's Hospital of Nanhai District,Foshan City,to explore the surgical techniques and postoperative complications of open gastrostomy.Results:All 31 patients successfully completed gastrostomy operation.Among them,3 patients had parastomal leakage and dermatitis after operation,3 patients had detubation in about 1 year,1 patient had a fistula blocked;1 patient had a fistula Rupture,1 case of embedding syndrome,1 case of local granulation tissue hyperplasia.After timely treatment,good clinical results were achieved.Conclusions:The use of PEG tube for gastrostomy for laparotomy is a simple and safe surgical method,with the combination of some advantages of endoscopic gastrostomy and laparotomy.It is worthy of popularization in primary hospitals.
