超声特征、病理分型等对乳腺恶性肿瘤超声BI-RADS分类评估的影响 被引量:8

摘要 目的探讨年龄、超声特征、病理分型、分子分型对乳腺恶性肿瘤超声乳腺影像报告和数据系统(BI-RADS)分类评估的影响。方法收集浙江大学医学院附属杭州市第一人民医院2015年1月至2018年6月经手术病理证实为乳腺癌的672例患者的临床资料,分析患者年龄、超声特征、病理分型、分子分型对乳腺恶性肿瘤BI-RADS分类的影响。结果不同年龄、超声特征、病理分型患者的病灶超声诊断符合率比较,差异均有统计学意义(均P<0.05),其中60岁及以上(86.10%)>40~59岁(82.54%)>40岁以下(65.09%),两者兼有(100.00%)>肿块型(84.73%)>非肿块型(60.98%),浸润性导管癌(88.28%)>其他病理类型(62.02%)>导管原位癌(56.99%);不同分子分型患者的病灶超声诊断符合率比较,差异无统计学意义(P>0.05)。结论年龄、超声特征、病理分型对乳腺恶性肿瘤BI-RADS分类有一定的影响,特别是低龄、超声特征为非肿块型、病理分型为导管原位癌的患者BI-RADS分类可能被低估。 Objective To investigate the influencing factors of BI-RADS-US categories for breast malignant tumors.Methods The clinical data and echographic findings of 672 patients with breast cancer confirmed by surgical pathology from January 2015 to June 2018 in Hangzhou First People’s Hospital,Zhejiang University School of Medicine were retrospectively analyzed.The effects of age,ultrasonic characteristics,pathological types and molecular types on the classification of BI-RADS of breast malignant tumor were analyzed.Results There significant differences in coincidence rates of ultrasound diagnosis with pathological diagnosis among patients with different ages,ultrasonic types and pathological types(all P<0.05).The coincidence rate in patients with age of 60 years old was highest(86.10%),followed by that in patients with age of 40-59 years(82.54%)and in patients under 40 years old(65.09%).The coincidence rate in both mass and non-mass like group was highest(100.00%),followed by that in masses group(84.73%)and in non-mass like group(60.98%).The coincidence rate in patients with invasive ductal carcinoma was highest(88.28%),followed by that with other pathological types(62.02%)and ductal carcinoma in situ(56.99%).There was no significant difference in the coincidence rate of ultrasound diagnosis among patients with different molecular types(P>0.05).Conclusion Age,ultrasonic characteristics and pathological classification have a certain effect on the BI-RADS-US classification of breast malignant tumors,especially the BI-RADS classification of patients with young age,ultrasound characteristics of non-mass like type,and pathological classification of ductal carcinoma in situ may be underestimated.
