运动干预对改善喉癌术后病人癌因性疲乏及生活质量的效果研究

Effect of exercise intervention on cancer-related fatigue and quality of life in postoperative patients with laryngeal cancer surgery

[目的]探讨有氧联合抗阻运动对喉癌术后病人癌因性疲乏(CRF)及生活质量的影响。[方法]选取2017年9月—2019年9月于福建省肿瘤医院行喉癌根治性切除术的82例病人为研究对象,采用随机数字表法分为运动组和对照组各41例。对照组给予常规的出院康复指导,运动组在常规护理基础上进行有氧联合抗阻运动。采用Piper疲乏修正量表(RPFS)、第4版癌症病人生命质量测定量表共性模块(Functional Assessment of Cancer Therapy-Generic Scale,FACT-G)评估两组病人出院时、干预后4周的CRF和生活质量评分。[结果]出院时两组病人生活质量和CRF总分及各维度得分比较差异均无统计学意义(P均>0.05),干预后4周运动组CRF总分及各维度得分均低于对照组,生活质量高于对照组(P均<0.05)。[结论]喉癌术后病人普遍存在CRF,有氧联合抗阻运动可有效减轻病人CRF程度,提高其生活质量。

Objective:To investigate the effect of aerobic exercise combined with impedance movement on cancer-related fatigue(CRF)and quality of life(QOL)in postoperative patients with laryngeal cancer surgery.Methods:A total of 82 patients who underwent radical resection of laryngeal cancer in Fujian Cancer Hospital from September 2017 to September 2019 were selected as the study subjects and randomly divided into exercise group and control group by using the random number table method,41 patients in each.The patients in control group received routine discharge rehabilitation guidance,while the patients in exercise group received aerobic exercise combined with impedance movement on the basis of routine nursing.The CRF and QOL scores at discharge and 4 weeks after intervention were assessed using the Revised Piper Fatigue Scale(RPFS)and Functional Assessment of Cancer Therapy-Generic Scale(FACT-G).Results:There was no statistically significant difference among age,gender,educational level,surgical procedure and pathological stage between the two groups(P>0.05).At discharge,the QOL and CRF total score and all dimensions scores in the two groups were poor,and the difference between the two groups was not statistically significant(P>0.05).After the intervention,the CRF total score and all dimensions scores of the exercise group were lower than those of the control group,while the QOL in the exercise group was higher than that of the control group(P<0.05).Conclusions:CRF was common in postoperative patients with laryngeal cancer surgery.Aerobic exercise combined with impedance movement could effectively improve the CRF level and QOL of patients.
