大剂量甲强龙联合持续气道正压通气对重症支气管哮喘患者血气分析及氧化应激指标的影响

Effects of high-dose methylprednisolone combined with continuous positive airway pressure ventilation on blood gas analysis and oxidative stress indexes in patients with severe bronchial asthma

目的:探讨大剂量甲强龙联合持续气道正压通气对重症支气管哮喘患者血气分析及氧化应激指标的影响。方法:2016年1月-2019年7月收治重症支气管哮喘患者84例,随机分为两组,各42例。对照组接受持续气道正压通气加常规治疗;观察组在持续气道正压通气加常规治疗的基础上加大剂量甲强龙。比较两组治疗前后血气分析指标及氧化应激指标。结果:观察组治疗后二氧化碳分压(PaCO2)、氧分压(PaO2)、超氧化物歧化酶(SOD)、脂质过氧化物(LPO)、谷胱甘肽过氧化物酶(GSH-Px)均优于对照组,差异均有统计学意义(P<0.05)。结论:大剂量甲强龙联合持续气道正压通气可改善重症支气管哮喘患者血气分析指标,也能纠正氧化应激失衡。

Objective:To investigate the effects of high-dose methylprednisolone combined with continuous positive airway pressure ventilation on blood gas analysis and oxidative stress indexes in patients with severe bronchial asthma.Methods:From January 2016 to July 2019,84 patients with severe bronchial asthma were selected,they were randomly divided into the two groups with 42 cases in each group.The control group received continuous positive airway pressure ventilation and routine treatment.The observation group was treated with high-dose of methylprednisolone on the basis of continuous positive airway pressure ventilation and routine treatment.The indexes of blood gas analysis and oxidative stress were compared between the two groups before and after treatment.Results:After treatment,the partial pressure of carbon dioxide(PaCO2),partial pressure of oxygen(PaO2),superoxide dismutase(SOD),lipid peroxide(LPO)and glutathione peroxidase(GSH-Px)in the observation group were significantly better than those in the control group,the differences were statistically significant(P<0.05).Conclusion:High-dose methylprednisolone combined with continuous positive airway pressure ventilation can improve the index of blood gas analysis and correct the imbalance of oxidative stress in patients with severe bronchial asthma.
