超声定量技术与脂肪肝指数相结合在非酒精性脂肪肝中的诊断价值研究

摘要 目的 探讨超声定量技术与脂肪肝指数相结合在非酒精性脂肪肝中的诊断价值.方法 选择2018年2月至2019年2月在某院进行体检的150例受试者为本次研究对象,其中68例诊断为非酒精性脂肪肝.所有受试者均进行超声定量检测和脂肪肝指数计算.以临床诊断结果为金标准,分析超声定量技术、脂肪肝指数单独及联合检测在非酒精性脂肪肝中的诊断效能.结果 超声定量技术、脂肪肝指数的诊断结果均为非酒精性脂肪肝诊断结果的独立影响因素.受试者工作特征曲线(ROC曲线)分析结果显示,超声定量技术、脂肪肝指数及联合预测因子在诊断非酒精性脂肪肝中的ROC曲线下面积分别为0.805、0.752、0.877,差异均有高度统计学意义(P<0.01).超声定量技术、脂肪肝指数联合检测用于非酒精性脂肪肝诊断的准确度、敏感度、特异度分别为88.00%、86.76%、89.02%,均高于超声定量技术、脂肪肝指数单独检测的结果.结论 将超声定量技术、脂肪肝指数联合检测用于非酒精性脂肪肝的诊断有助于提升诊断的敏感度和特异度. Objective To explore the diagnostic value of ultrasonic quantitative technique combined with fatty liver index in nonalcoholic fatty liver disease.Methods A total of 150 subjects who underwent physical examination in a hospital from February 2018 to February 2019 were selected as the subjects of this study,68 of whom were diagnosed with non-alcoholic fatty liver disease.All subjects underwent quantitative ultrasound detection and fatty liver index calculation.With the clinical diagnosis results as the gold standard,the diagnostic efficacy of ultrasound quantitative technique,fatty liver index alone and combined in non-alcoholic fatty liver was analyzed.Results Both ultrasonic quantitative technique and fatty liver index were independent factors influencing the diagnosis of nonalcoholic fatty liver disease.The results of receiver operating characteristic(ROC)curve analysis showed that the area under ROC curve of ultrasonic quantitative technique,fatty liver index and combined predictor in the diagnosis of non-alcoholic fatty liver was 0.805,0.752 and 0.877,respectively,which were statistically significant(P<0.01).The accuracy,sensitivity and specificity of ultrasonic quantitative technology and fatty liver index combined detection for non-alcoholic fatty liver were 88.00%,86.76%and 89.02%,respectively,which were all higher than the results of respective detection by ultrasonic quantitative technology or fatty liver index alone.Conclusion The combination of ultrasonic quantification technology and fatty liver index in the diagnosis of non-alcoholic fatty liver is helpful to improve the sensitivity and specificity of the diagnosis.
