高脂血症骨折患者发生下肢深静脉栓塞的危险因素分析

Analysis of Risk Factors of Deep Vein Thrombosis in Patients with Hyperlipidemia Fractures

目的探究导致高脂血症骨折患者发生下肢深静脉栓塞的危险因素。方法选取在2018年6月—2019年6月该院收治的70例高脂血症骨折并发下肢深静脉栓塞患者(观察组),并选取同期70例没有并发下肢深静脉栓塞的高脂血症骨折患者(对照组),对导致高脂血症骨折患者发生下肢深静脉栓塞的危险因素进行积极探究。结果观察组多部位骨折(60.00%)、超过3年的静脉曲张史(58.57%)、骨科手术(65.71%)、冠心病史(85.71%)、高血压史(72.86%)、糖尿病史(54.29%)、吸烟史(88.57%)及年龄超过60岁(34.29%)的比率均高于对照组,差异有统计学意义(χ^2=11.622、10.628、11.329、10.251、14.125、11.036、10.095、11.215,P<0.05);通过多因素回归分析显示,导致高脂血症骨折患者发生下肢深静脉栓塞的独立危险因素包括多部位骨折、超过3年的静脉曲张史、骨科手术及年龄超过60岁。结论导致高脂血症骨折患者发生下肢深静脉栓塞的危险因素十分复杂,骨折后3 d内为下肢深静脉栓塞发生的主要集中时间,左下肢是主要的发病肢体,而对于高脂血症骨折患者来说,多部位骨折、超过3年的静脉曲张史、骨科手术及年龄超过60岁是影响其发生下肢深静脉栓塞的主要危险因素。

Objective To investigate the risk factors of lower extremity deep vein embolism in patients with hyperlipidemia fractures.Methods From June 2018 to June 2019,70 patients with hyperlipidemia fractures complicated with deep vein thrombosis of the lower extremity(observation group)were selected,and 70 cases of hyperlipidemia fracture patients(control group)without concurrent deep vein thrombosis of the lower extremity during the same period were selected,actively explored the risk factors of lower extremity deep vein embolism in patients with hyperlipidemia fractures.Results Multi-site fractures(60.00%),varicose veins history(58.57%),orthopedic surgery(65.71%),coronary heart disease history(85.71%),hypertension history(72.86%),and diabetes history(54.29%),smoking history(88.57%)and age over 60(34.29%)in the observation group,were higher than those in the control group,and the difference was statistically significant(χ^2=11.622,10.628,11.329,10.251,14.125,11.036,10.095,11.215,P<0.05);Multivariate regression analysis showed that the independent risk factors for deep vein embolism in patients with hyperlipidemia fractures included multisite fractures,a history of varicose veins over 3 years,orthopedic surgery,and age over 60 years.Conclusion The risk factors for lower extremity deep vein embolism in patients with hyperlipidemia fractures are very complicated.The main concentration time of lower extremity deep vein embolism is within 3 days after fracture.The left lower limb is the main diseased limb.For patients with hyperlipidemia fractures,multi-site fractures,a history of varicose veins of more than 3 years,orthopedic surgery,and age over 60 are the main risk factors affecting the occurrence of deep vein thrombosis of the lower limbs.
