摘要 目的探讨基质细胞衍生因子-1(SDF-1)与血液透析(HD)患者炎症及营养标志物的相关性。方法纳入152例于2013年1月至2016年12月在南方医科大学珠江医院开始接受透析治疗的慢性肾病患者为HD组,以同期该院健康体检中心50例健康志愿者作为对照组。酶联免疫吸附测定检测患者血清基质金属蛋白酶(MMP)-2、MMP-9、髓过氧化物酶(MPO)、骨桥蛋白(OPN)及SDF-1含量;老年营养风险指数(GNRI)采用公式:1.489×白蛋白+41.7×(体质量/理想体质量)计算。结果与对照组相比,HD组SDF-1与MPO较高(P<0.05);但OPN、MMP-2与MMP-9较低(P<0.05);SDF-1浓度与MMP-2、MMP-9、MPO及OPN正相关;与白蛋白、GNRI分数及标准化蛋白质分解率呈负相关。结论 SDF-1与血液透析患者炎症及营养状态密切相关。 【Objective】 To explore the correlation between stromal cell-derived factor 1(SDF-1) and inflammation and nutritional markers of hemodialysis(HD) patients. 【Methods】 From January 2013 to December 2016, 152 HD patients were enrolled and received follow up until December 2019. Matrix metalloproteinase-2/-9(MMP-2/-9), myeloperoxidase(MPO),osteopontin and SDF-1 were measured by enzyme linked immunosorbent assay. The Geriatric Nutritional Risk Index(GNRI) score was calculated by modifying the Nutritional Risk Index for elderly patients based on the following formula: GNRI=[14.89*albumin(g/dL)] + [41.7*(body weight/ideal body weight)]. Fifty persons with normal renal function were enrolled as a control group.【Results】 The mean age of the subjects in the control group and the hemodialysis group was 51.2±8.3 and 53.2±6.2 years,respectively. Compared with the control group, stromal cell derived factor 1(58.7±14.2 pg/mL vs. 40.6±16.3 pg/mL, P=0.023) and myelperoxidase(65.3±5.8 pg/mL vs. 53.7 ±5.2 pg/mL, P=0.013) were higher in the hemodialysis group. However, osteopontin(22.6±3.8 ng/mL vs. 89.2±10.2 ng/mL, P=0.001), metalloproteinase-2(483.3±39.1 ng/mL vs. 569.7±68.1 ng/mL, P=0.001) and metalloproteinase-9(328.6±28.9 ng/mL vs. 1270.6±289.1 ng/mL, P=0.001) were lower. The concentration of stromal cell derived factor-1 was positively correlated with MMP-2(r=0.312, P=0.001), MMP-9(r=0.193, P=0.021), myeloperoxidase(r=0.532, P=0.001), and osteopontin(r=0.317, P=0.001). It was negatively correlated with albumin(r=-0.192, P=0.018), GNRI fraction(r=-0.227, P=0.007)and standardized protein catabolic rate(r=-0.216, P=0.012).【Conclusion】SDF-1 may have a role in influencing the inflammation and nutrition status of HD patients but further studies are necessary to investigate this possibility.
