摘要
目的研究白细胞介素6(IL-6)基因多态性对老年严重心力衰竭患者预后的预测作用。方法选择心力衰竭患者160例为观察组,健康体检者160例为对照组,采用PCR提取基因组的DNA,检测IL-6基因174、634、572位点多态性。结果2组IL-6174位点基因多态性比较,无统计学差异(P>0.05);观察组IL-6634位点、572位点CC基因型(65.00%vs 76.25%,33.12%vs 61.25%,P<0.01)及C等位基因频率(76.88%vs 86.87%,51.56%vs 77.50%,P<0.01)明显低于对照组。不同程度心力衰竭患者IL-6174位点基因多态性和C等位基因频率比较无统计学差异(P>0.05),轻度、中度患者IL-6634位点和572位点基因多态性比较,无统计学差异(P>0.05)。重度患者IL-6634、572位点CC基因型、C等位基因频率明显低于轻度和中度患者(P<0.05)。结论IL-6174位点多态性与严重心力衰竭无明显相关,但随着心力衰竭的加重,IL-6634、572位点CC基因型、C等位基因表达明显减少,可通过634、572位点基因的多态性表达预测严重心力衰竭的预后。
Objective To study the role of IL-6 gene polymorphism in predicting the outcome of elderly severe heart failure(HF)patients.Methods One hundred and sixty HF patients served as an observation group with 160 persons who underwent physical examination in our hospital served as a control group.The peripheral venous blood sample was taken to extract genomic DNA by PCR for detecting the IL-6 gene polymorphism at the sites of 174,634 and 572.Results No significant difference was detected in IL-6 gene polymorphism at the site of 174(P>0.05).The frequency of IL-6 CC genotype and allele C gene at the sites of 634 and 572 was significantly lower in observation group than in control group(65.00%vs 76.25%,33.12%vs 61.25%,P<0.01;76.88%vs 86.87%,51.56%vs 77.50%,P<0.01).No significant difference was detected in IL-6 gene polymorphism at the sites of 174,634 and 572 in mild and moderate HF patients(P>0.05).The frequency of IL-6 CC genotype and allele C gene at the sites 634 and 572 was significantly lower in severe HF patients than in mild and moderate HF patients(P<0.05).ConclusionIL-6 gene polymorphism at the site of 174 is not closely related with severe HF.However,the expression of IL-6 CC genotype and C allele at the sites of 634 and 572 decreases significantly with the aggravating HF,which can thus predict the outcome of severe HF in the elderly.
