加味仙方活命饮防治卡培他滨相关性手足综合征疗效观察

Curative Effect Observation of Modified Xianfang Huoming Yin on Capecitabine-related Hand-Foot Syndrome

目的应用加味仙方活命饮防治卡培他滨化疗方案(单药及联合用药)所致的手足综合征(HFS),客观评价其防治疗效,为HFS的临床治疗提供方法学借鉴。方法通过SPSS估算样本量,纳入北京中医药大学第三附属医院肿瘤血液科的患者30例,30例均符合卡培他滨相关性HFS的收纳标准;采用治疗前后患者自身对照的方法,收集给药干预前后HFS临床分级、疼痛症状评分、皮肤不良反应事件,从临床分级变化、疼痛改善情况、皮肤不良反应发生率3个方面,评价加味仙方活命饮的临床防治疗效及安全性。结果(1)HFS临床分级变化:治疗后18例(18/30)患者临床分级下降,其中7例(7/30)完全缓解;总有效率为60%,完全缓解率为23.33%;治疗前后具有统计学差异(P<0.05)。(2)疼痛症状变化:治疗后27例(27/30)患者疼痛症状改善,其中11例(11/30)完全缓解;总有效率为90%,完全缓解率为36.67%;治疗前后具有显著统计学差异(P<0.01)。(3)皮肤不良反应事件:治疗后30例患者均未出现皮肤过敏、皮疹等不良反应。结论加味仙方活命饮可有效降低卡培他滨相关性HFS的临床分级,缓解患者疼痛症状,且安全性高,具有一定的临床应用价值。

Objective Modified Xianfang Huoming Yin was used to prevent and treat hand-foot syndrome caused by capecitabine chemotherapy regimen(single drug and combination drug),and its therapeutic effect was evaluated objectively,which could provide methodological reference for the clinical treatment of HFS.Methods The sample size was estimated by SPSS,and 30 patients were included in the department of oncology and hematology,third affiliated hospital of Beijing university of Chinese medicine.All 30 patients met the admission criteria of capecitabine-related HFS.The clinical grading,pain symptom score and skin adverse reaction events of HFS before and after the intervention were collected by self-control method.The clinical efficacy and safety of Modified Xianfang Huoming Yin were evaluated from three aspects:changes of clinical grading,improvement of pain and incidence of skin adverse reaction.Results(1)Changes in clinical grading of HFS:After treatment,clinical grading of 18 patients(18/30)decreased,of which 7 patients(7/30)had complete remission;the total effective rate was 60%,and the complete remission rate was 23.33%.There was statistical difference before and after treatment(P<0.05).(2)Changes of pain symptoms:27 patients(27/30)improved their pain symptoms after treatment,11 of them(11/30)completely alleviated;the total effective rate was 90%,and the complete remission rate was 36.67%.There was significant statistical difference before and after treatment(P<0.01).(3)Adverse skin reactions:After treatment,no skin allergies,rashes and other adverse reactions occurred in 30 patients.Conclusion Modified Xianfang Huoming Yin can effectively reduce the clinical classification of capecitabine-related HFS,relieve pain symptoms of patients,and has high safety,which has certain clinical application value.
