超声心动图及二维超声斑点追踪显像技术评价通心络胶囊对急性广泛前壁心肌梗死PCI术后患者左室功能作用研究

Evaluation of the Effect of Tongxinluo Capsule for Left Ventricular Function in Acute Anterior Wall Infarction Patients Treated with PCI by Echocardiography and Two-dimensional Speckle Tracking Technology

目的采用超声心动图及二维超声斑点追踪显像技术(STI)评价通心络胶囊对急性广泛前壁心肌梗死急诊经皮冠状动脉介入术(PCI)术后患者的左室整体及局部收缩功能变化,探讨通心络胶囊对改善PCI术后左室功能作用的研究。方法对72例AMI患者在术后72 h及1个月进行了超声心动图及STI检查,检测左室整体收缩功能、舒张功能以及梗死心肌、缺血心肌的局部收缩功能。结果(1)通心络+PCI常规组和常规PCI组在PCI术后72 h与1个月后对比梗死心肌、缺血心肌、正常心肌纵向应变SL均明显改善(P值分别为0.001、0.000、0.000、0.000、0.047、0.000、0.000、0.000,均<0.05),两组EF值术后明显提高(P值分别为0.015、0.00,均<0.05),整体收缩功能及舒张功能差异无统计学意义(均P>0.05)。(2)术后1个月比较,通心络+PCI常规组相对于PCI常规组缺血心肌SL显著增大(P=0.025<0.05)。(3)术后72 h比较,通心络+常规组与PCI常规组心脏左室功能各参数均差异无统计学意义(均P>0.05)。结论急性广泛前壁心肌梗死患者PCI术后在规范口服西药的基础上加用通心络胶囊,可改善缺血心肌收缩功能,提高短期疗效(1个月)。

Objective Echocardiography and two-dimensional speckle tracking imaging(STI)were used to evaluate the effect of Tongxinluo Capsule on left ventricular systolic function in patients with acute extensive anterior myocardial infarction after PCI,to study the effect of Tongxinluo Capsule on improving left ventricular function after PCI.Methods The selected 72 patients who suffered acute myocardial infarction had taken PCI.Then checking the systolic and diastolic function of the whole left ventricle and the partial systolic function of the infracted and ischemia myocardium via ultrasonic cardiogram within 72 hours and one month after PCI.Results(1)After 72 hours and one month after PCI operation,compared with the PCI routine group,The longitudinal strain SL of infarction,ischemia and normal myocardium in the Tongxinluo Capsule and PCI routine group had been significantly improved(P=0.001,0.000,0.000,0.000,0.047,0.000,0.000,0.000<0.05).There was no obvious differences between the whole systolic and diastolic function(P=0.015,0.00<0.05).(2)One month after PCI operation,compared with PCI routine group,the SL of ischemia myocardium in the Tongxinluo Capsule and PCI routine group was significantly increased(P=0.025<0.05).(3)72 hours after PCI operation,compared with PCI routine group,all the parameters of left ventricle function in the Tongxinluo Capsule and PCI routine group had no significantly differences(all P>0.05).Conclusion Patients who suffered acute myocardial infarction after PCI operation had orally taken the medicine regularly added Tongxinluo Capsule treatment,the short term effect(within one month)could improve the systolic function of the infarction and ischemia myocardium.But there's no significantly improvement of the acute period(72 hours after PCI).Two-dimensional speckle tracking technology which could check systolic function of myocardium is the powerful supplement of the routine ultrasound.
