呼吸道感染患儿肺炎支原体抗体检测及耐药性分析 被引量:2

摘要 目的小儿支原体肺炎仅通过临床症状及白细胞值诊断比较困难,进行病原学诊断可减少误诊或漏诊。本研究对628例呼吸道感染患儿肺炎支原体抗体检测及耐药性进行分析,为临床诊断及有效治疗提供科学依据。方法选取2017-01-01-12-31来宾市妇幼保健院儿科门诊及住院治疗的628例呼吸道感染患儿为研究对象,进行血常规检测的同时检测肺炎支原体抗体,阳性者做药敏试验。结果628例呼吸道感染患儿肺炎支原体抗体阳性率为44.11%(277/628),抗体阳性率随年龄增长而升高,各年龄段比较差异有统计学意义,χ^2=12.456,P<0.01。女性感染率为48.70%,高于男性的40.67%,差异有统计学意义,χ^2=4.075,P<0.05。感染患儿药敏试验结果显示,敏感性从高到低依次为司帕沙星(95.31%)、强力霉素(95.31%)、左氧氟沙星(94.22%)、美满霉素(93.86%)、阿奇霉素(91.34%)、克拉霉素(71.12%)、罗红霉素(64.62%)、氧氟沙星(38.27%)和交沙霉素(20.58%),对交沙霉素耐药率最高,为58.48%。结论肺炎支原体抗体阳性率随年龄增长而升高,女性高于男性,对司帕沙星、强力霉素和左氧氟沙星等抗生素有较强敏感性,而对氧氟沙星和交沙霉素敏感性较差。 OBJECTIVE Mycoplasma pneumonia in children is difficult to diagnose only by clinical symptoms and leukocyte values,and etiological diagnosis can reduce misdiagnose or miss the diagnosis.This study analyzes the detection and drug resistance of Mycoplasma pneumoniae in 628 children with respiratory infections,and provides a scientific basis for clinical diagnosis and effective treatment.METHODS A total of 628 children with respiratory infections in paediatric outpatient and inpatient treatment at Laibin Maternal and Child Health Hospital from January 01 to December 31,2017,were selected as the research subjects.Mycoplasma pneumoniae antibodies were detected at the same time as routine blood tests,and the positives were tested for drug sensitivity.RESULTS The positive rate of Mycoplasma pneumoniae antibodies in 628 children with respiratory tract infection was 44.11%(277/628).The antibody positive rate increased with age,and there was a statistically significant difference between different age groups,χ^2=12.456,P<0.01.The infection rate was 48.70%in women,which was higher than that of the men(40.67%),effectively,χ^2=4.075,P<0.05.The results of drug sensitivity tests in children with infection showed that the order of sencitivity from high to low was sparfloxacin(95.31%),doxycycline(95.31%),levofloxacin(94.22%),minocycline(93.86%),azithromycin(91.34%),clarithromycin(71.12%),roxithromycin(64.62%),ofloxacin(38.27%)and josamycin(20.58%)it had the highest resistance to josamycin(58.48%).CONCLUSIONS The positive rate of Mycoplasma pneumoniae antibodies increased with age,and women were higher than men.They were more sensitive to antibiotics such as spafloxacin,doxycycline,and levofloxacin,but more sensitive to ofloxacin and josamycin.
