青光眼白内障联合手术不同切口对角膜内皮细胞的影响

Effect of glaucoma cataract combined with different incisions on corneal endothelial cells

目的探讨手术不同切口治疗青光眼合并白内障对患者角膜内皮细胞的影响。方法我院于2018年1月至2019年8月期间收治的闭角型青光眼合并白内障患者中,从中选择了114例作为本次研究的病例,其中单一切口组与双切口组的病例数均为57例,两组均接受小梁切除联合超声乳化手术。结果对两组治疗后的效果进行评价,单一切口组患者的视力为(4.14±1.23),眼压为(14.21±1.07)mmHg,双切口组患者视力为(4.41±1.12),眼压为(15.35±5.27)mmHg,两组患者的视力及眼压改善情况无明显差异(P>0.05);两组患者术前角膜内皮细胞计数分分别为(2213.21±176.83)个/m^2、(2187.95±196.34)个/m^2,比较差异无统计学意义(P>0.05),术后双切口组患者角膜内皮密度值为(1743.18±189.37)个/m^2,显著低于单一切口组的(1929.53±174.24)个/m^2(P<0.05)。结论对青光眼合并白内障患者应用单一切口的小梁切除术联合超声乳化术治疗,效果确切,患者角膜内皮细胞损失较少,值得推广。

Objetive To investigate the effect of different incisions on corneal endothelial cells in patients with glaucoma and cataract.Methods From January 2018 to August 2019,114 patients with angle-closure glaucoma complicated with cataract were selected as the cases in this study.These patients were divided into single incision group and double incision group,each group had 57 cases,and both groups received trabeculectomy combined with phacoemulsification.Results In the single incision patient group,the visual acuity was(4.14 ±1.23) and intraocular pressure was(14.21±1.07) mmHg.And the visual acuity was(4.41±1.12) and intraocular pressure was(15.35 ±5.27) mmHg in the double incision patient group.But there was no significant difference between the two groups(P> 0.05).The preoperative corneal endothelial cell counts of the two groups were(2213.21±176.83)/M^2 and(2187.9±5196.34)/M^2,respectively(P> 0.05),The corneal endothelium density was(1743.18 ±189.37)/M^2 in the double-incision group,significantly lower than that in the single-incision group(1929.53 ±174.24)/M±2(P <0.05).Conclusion Single incision trabeculectomy combined with phacoemulsification is effective in treating glaucoma patients with cataract.
